Abstract
Aims: Distal radioulnar joint (DRUJ) instability may occur after distal radius fractures and can negatively affect wrist function. Temporary transulnar pinning is commonly used to provide mechanical stabilization and allow soft tissue healing; however, its effect on clinical outcomes remains controversial. This study aimed to evaluate the clinical outcomes of temporary transulnar pinning in patients with distal radius fractures accompanied by ulnar styloid fractures and DRUJ instability. Methods: In this retrospective study, patients with Association for the Study of Internal Fixation/Orthopaedic Trauma Association type C distal radius fractures accompanied by ulnar styloid fractures and suspected intraoperative DRUJ instability were included. All fractures were treated with volar plate fixation. Patients were divided into two groups: temporary transulnar pinning with long-arm splinting (n=14) and long-arm splinting alone (n=32). Clinical outcomes were evaluated using the Visual Analog Scale (VAS), quick disabilities of the arm, shoulder and hand (QuickDASH) score, grip strength, and wrist range of motion at final follow-up. Results: Forty-six patients were included. No significant differences were observed between the groups regarding demographic characteristics or radiological parameters. At final follow-up, VAS scores, QuickDASH scores, grip strength, and range of motion were similar between groups (p>0.05). No complications requiring revision surgery were observed. Conclusion: Temporary transulnar pinning did not provide superior clinical outcomes compared with conservative immobilization in patients with distal radius fractures accompanied by ulnar styloid fractures and DRUJ instability.
