Abstract
Aim: The aim of this study was to determine the incidence of inadvertent hypothermia and related factors associated with hypothermia in geriatric orthopaedic patients and to evaluate the findings of hypothermia. Method: The descriptive study was conducted with 155 patients aged 65 years and older who underwent surgical treatment in the Orthopaedic and Traumatology Clinic of a tertiary health care institution in Mersin. Data were collected between March and August 2018 using a data collection form prepared in line with the literature. Descriptive statistics and chi-square tests were used in data analysis. Statistical significance was accepted as p<0.05. Results: The mean age of the patients was 75.24 years, 57.4% were male, and main reasons for admission to the orthopaedic clinic were hip fracture (27.1%) and gonarthrosis (26.5%). The rate of inadvertent hypothermia in the preoperative period was between 0.6-4.5%. After surgery, 40.6% were admitted to the post-anesthesia care unit (PACU) and 23.9% to the clinic as hypothermic. The accompanying findings of hypothermia were chills, shivering, and piloerection. The rate of hypothermia was higher in patients with fasting time ≥ 11 hours, duration of surgery ≥ 90 minutes, and at least one chronic disease (p<0.05). Hypothermia continued in the clinic in 97.3% (p<0.001) of the patients who were hypothermic in the PACU and in 75.7% (p=0.010) of those who were not actively warmed. Conclusion: The results of the study showed that the rate of inadvertent hypothermia in geriatric orthopaedic patients was high in the early postoperative period. The factors associated with inadvertent hypothermia were as follows: fasting time before surgery and longer duration of surgery, having at least one chronic disease, being hypothermic on admission to the PACU and lack of active heating in this unit. These results show the importance of actively warming geriatric orthopaedic patients during the surgery and taking into account the duration of fasting, duration of surgery and the presence of chronic diseases when planning hypothermia prevention interventions.
