Anesthetic Management of a Patient with Behçet’s Disease Undergoing Beating Heart Coronary Artery Bypass Grafting

Abstract
General anesthesia was planned for elective beating heart coronary artery bypass grafting in a 32-year-old patient with Behçet’s disease receiving colchicine and salazopyrin therapy for 13 years. After consultation with neurologists, dermatologists, and internal medicine physicians, the colchicine and salazopyrin therapy was terminated a week before surgery. The surgery was performed with the beating heart technique following standard sternotomy and heparinization. Postoperatively, there were no new lesions of the skin and oropharynx due to invasive procedures. The patient was discharged without any complications on postoperative day 4. In conclusion, the choice of the anesthetic method and medications as well as postoperative follow-up beating heart coronary artery bypass grafting can be safely performed in patients with Behçet’s disease via meticulous preoperative evaluations and consultations.
Özet
Otuz iki yaşında, 13 yıldır kolşisin ve salozoprin tedavisi alan Behçet hastasına elektif şartlarda atan kalpte koroner arter baypas ameliyatı için genel anestezi planlandı. Nöroloji, dermatoloji ve dahiliye bölümlerince konsülte edilerek kolşisin ve salozoprin tedavisi operasyondan bir hafta önce kesildi. Operasyon standart sternotomi ve heparinizasyonu takiben atan kalpte gerçekleştirildi. Postoperatif dönemde invaziv girişimlere ve entübasyona bağlı ciltte ve orofarenkste yeni lezyonlara rastlanmadı. Hasta postoperatif 4. gün komplikasyonsuz olarak taburcu edildi. Sonuç olarak, Behçet hastalarında anestezi öncesi yapılan dikkatli bir preoperatif değerlendirme, istenen konsültasyonlar, seçilen anestezi yöntemi ve ilaçlar ve postoperatif takip ile atan kalpte koroner arter baypas operasyonu güvenle uygulanabilir.
