Determination of the Use of Nonpharmacologic Methods by Nurses for Postoperative Pain Management

Abstract

Introduction: Postoperative pain is an acute condition that arises from the stimulation of nociceptors as a result of surgical trauma and ends with tissue healing. Pain relief is very important for the relief of the individual, the increase in the quality of life and the reduction of complications.Aim: The aim of the study was to determine the use of nonpharmacological methods of the nurses in the management of postoperative pain.Method: This descriptive study was conducted with 538 nurses working in surgical clinics of three public and three private hospitals in Kocaeli between June and September 2017. The data were collected by using “The Non-Pharmacological Nursing Interventions Form for Pain” prepared by the researchers in accordance with the literature. SPSS 21.0 package program was used for data analysis.Results: The nonpharmacological methods frequently used by nurses were found to be positioning (51.3%), imagination (46.8%), relief (45.7%) and hot-cold application (43.5%). Insufficient number of nurses (68.0%) and heavy workload (64.7%) were found to be the most common reasons for not applying non-pharmacological methods. There was a statistically significant difference between all nonpharmacological methods used according to education level and working time of nurses (p <0,05). Conclusion: It was found that approximately half of the nurses sometimes used massage method, half of them frequently used positioning and hot-cold application methods, less than half of the nurses sometimes used the therapeutic touch method, the majority of nurses never used transcutaneous electrical nerve stimulation management.

Özet

Giriş: Ameliyat sonrası ağrı, cerrahi travma sonucu nosiseptörlerin uyarılmasıyla ortaya çıkan ve doku iyileşmesi ile sona eren akut bir durumdur. Ağrının kontrol altına alınması bireyin rahatlaması, yaşam kalitesinin yükselmesi ve komplikasyonların azalması açısından oldukça önemlidir. Amaç: Araştırmanın amacı hemşirelerin ameliyat sonrası ağrı yönetiminde nonfarmakolojik yöntemleri kullanma durumlarını belirlemektir. Yöntem: Tanımlayıcı tipte olan bu araştırma, Haziran- Eylül 2017 tarihleri arasında Kocaeli ilindeki üç kamu ve üç özel hastanenin cerrahi kliniklerinde çalışan toplam 538 hemşire ile gerçekleştirildi. Verilerin toplanmasında araştırmacı tarafından literatür bilgisi doğrultusunda oluşturulan “Ağrıya Yönelik Nonfarmakolojik Hemşirelik Girişimlerini Belirleme Formu” kullanıldı. Verilerin değerlendirilmesinde SPSS 21.0 paket programı kullanıldı. Bulgular: Hemşirelerin sık sık uyguladıkları nonfarmakolojik yöntemlerin pozisyon verme (%51,3), düşleme (%46,8), rahatlatma (%45,7) ve sıcak-soğuk uygulama (%43,5) olduğu saptandı. Ameliyat sonrası nonfarmakolojik yöntemleri uygulamama nedenleri arasında ise en sık hemşire sayısının yetersiz olması (%68,0) ve ağır iş yükü (%64,7) olduğu belirlendi. Hemşirelerin eğitim durumu ve çalışma süresi ile kullanılan tüm nonfarmakolojik yöntemler arasında istatistiksel olarak anlamlı bir farkın olduğu görüldü (p<0,05). Sonuç: Hemşirelerin yarısına yakının bazen masaj, yarısının sık sık pozison verme ve sıcak-soğuk uygulama, yarısından azının bazen terapötik dokunma yöntemini kullandığı, büyük çoğunluğunun ise transkütan elektriksel sinir uyarımı yöntemini hiçbir zaman kullanmadığı sonucu elde edildi.
