The Use of Invasive Tools in Intensive Care Units and Infection in The State Hospital

Abstract
Aim: There have been many invasive interventions on the patients in intensive care units. The applied invasive intervention, the medication used and the duration of staying affect the liability to cross (nosocomial) infection. It was aimed in this study to present the current situation of Burdur State Hospital by comparing the surveillance data related to invasive device from the patients monitored at intensive care units of the hospital to the national data and other studies. Material and Method: In the study, infections related to invasive device usage and invasive device usage rate were evaluated. The data was obtained from the Cross (Nosocomial) Infection Surveillance Network of Ministry of Health (INFLINE). The evaluation of data was carried out separately for surgical, internal and coronary intensive care units. Results: Rates of medical service related infection determined in the internal, surgical and coronary intensive care units of the hospital were 5.26, 4.89 and 0.00 respectively. The most frequent used invasive device in these units was determined as urinary catheter. The rates for the internal, surgical and coronary units were 0.95, 0.93 and 0.32 respectively. The infection rate related to the usage of invasive device in surgical and internal intensive care units was detected as VIP (6.44 and 13.9). Conclusion: The activity reports of infection control committee should be prepared with objective criteria. The pursuit of these reports is crucial to reduce cross (nosocomial) infections.

Amaç: Yoğun bakım ünitelerine yatan hastalara pek çok invaziv girişim yapılmaktadır. Hastane enfeksiyonunu, uygulanan invaziv girişim, kullanılan ilaçlar ve kalış süreleri etkilemektedir.Bu çalışmada, Burdur Devlet Hastanesi yoğun bakım ünitelerinde takibi yapılan hastaların, invaziv araç ilişkili sürveyans verileri ulusal veriler ve diğer çalışmalarla karşılaştırılarak hastanenin mevcut durumunun ortaya konulması amaçlanmıştır.Materyal ve Metod: İnvaziv araç kullanımı ile ilişkili enfeksiyon ve invaziv alet kullanım oranları değerlendirilmiştir. Verileri T.C. Sağlık Bakanlığı Hastane Enfeksiyonları Sürveyans ağı (İNFLİNE) yazılım programından alınmıştır. Verilerin değerlendirilmesi cerrahi, dahili ve koroner yoğun bakım üniteleri için ayrı olarak yapılmıştır.Bulgular: Hastanemizde bulunan yoğun bakım ünitelerinde tespit edilen sağlık hizmetleri ilişkili enfeksiyon hızları sırasıyla, dahili 5.26; cerrahi 4.89 ve koroner 0.00 olarak tespit edildi. Yoğun bakım ünitelerinde invaziv alet kullanım oranı en sık olarak üriner kateter olarak tespit edildi. Bunlar sırasıyla dahili (0.95), cerrahi (0.93) ve koroner (0.32) yoğun bakım ünitelerindeydi. Cerrahi ve dahili yoğun bakım ünitesinde invaziv alet kullanımı ile ilişkili enfeksiyon hızı VİP (6.44 ve 13.9) olarak tespit edildi. Sonuç: Enfeksiyon kontrol komitesi faaliyet raporları objektif kriterler ile hazırlanmalıdır. Bu raporların takibi hastane enfeksiyonlarının azaltılmasında önemlidir.
