Stem cell transplantation in multiple sclerosis: Experiences, questions and problems

Abstract

ABSTRACT Stem Cell Transplantation in Multiple Sclerosis: Experiences, Questions and Problems Scientific background: Autologous haematopoietic stem cell transplantation is an immunosuppressive treatment used to sustain longterm stabilization of multiple sclerosis (MS). The clinical efficacy of this therapy needs to be proved by studies comparing its effect with other treatments formerly shown to be effective in MS, such as mitoxantrone. High dose immunosuppressive treatment'together with haematopoietic stem cell transplantation has high mortality risk. For this reason, before suggesting these treatments to the patients we should think carefully. Conclusion: Although, MS cases with low EDSS scores and gadoliniumenhancement in brain MRI, and young patients with proggressive disease course are suggested as the most suitable candidates for haematopoietic stem cell transplantation in medical literature, it must be remembered that there is no study comparing its clinical efficacy and efficiency with the formerly approved drugs to treat MS.

Bilimsel zemin: Otolog hematopoetik kök hücre transplantasyonu, uzun süreli MS stabilizasyonu sağlanması amacıyla denenmekte olan immünsupressif bir tedavi yöntemidir. Bu tedavinin klinik yararlılığının, etkinliği gösterilmiş olan mitoksantron gibi mevcut standart tedaviler ile karşılaştırılarak doğrulanması gerekmektedir. Hematopoetik kök hücre transplantasyonu ve yanı sıra uygulanan yüksek doz immünsupressif tedaviler, beraberinde yüksek mortalite riskini de getirmektedir. Bu nedenle hastalara önermeden önce tüm yönlen dikkatle düşünülmelidir. Yorum: Literatürde; düşük özürlülük derecesi ile birlikte beyinde gadolinyum kontrastlanması gösteren, ilerleyici tipte hastalık seyri gözlenen genç hastalar, hematopoetik kök hücre transplantasyonu için en uygun adaylar , olarak önerilmekle birlikte, bu tedavileri etkinliği ispatlanmış tedavilerle karşılaştıran ve etkinliğini-üstünlüğünü ispatlayan bir çalışmanın henüz bildirilmediğini unutmamak gereklidir.
