Fertility and obstetric outcomes following myomectomies: Two case reports and review of the literature

Abstract
Objective: To present two cases with primary infertility whose pregnancies following myomectomy resulted in healthy infants by cesarean section, and to discuss fertility and obstetric outcomes following myomectomies. Materials and Methods: Two cases who were primary infertile for 14 and 9 years respectively, and whose infertility were unexplained except having intramural fibroids had myomectomies by laparotomy. One of the cases delivered at term while the other delivered at 32nd week of pregnancy, both by cesarean section. We review the literature, and discuss fertility and obstetric outcomes following myomectomy. Conclusion:Treatment of intramural fibroids of infertile patients without associated other infertility factors is myomectomy. Eventhough the risk of uterine rupture during pregnancy is small, cesarean section is recommended for delivery.
Özet
Amaç: Myomektomi sonrası gebelikleri sezaryen doğum ile sonlandırılan iki primer infertil olguyu sunmak ve myomektomi sonrası fertilite ve obstetrik sonuçları tartışmak. Materyal ve Metod: Sırasıyla 14 ve 9 yıllık primer infertil olan ve tetkiklerinde başka infertilite nedeni saptanmayan iki olguya laparotomi ile myomektomi uygulandı. Daha sonra gebe kalan iki hastadan ilki termde, ikincisi gebeliğin 32. haftasında iken sezaryen doğum ile sonlandırıldı. Myomektomi sonrası fertilite ve obstetrik sonuçlar literatür bilgileri ışığında tartışıldı. Sonuçlar:Başka infertilite nedeni bulunmayan infertil hastalarda, intramural veya kaviteyi etkileyen myomların tedavisi myomektomidir. Myomektomi sonrası düşük uterus rüptürü riskine rağmen, doğumun sezaryen seksiyo ile sonlandırılması önerilmektedir.
