Abstract

Pharmacological agents are choosen for the treatment of postoperative shivering. Clinical studies are going on to find perfect agent with low side effects. In our study we aimed to investigate the effects of dexmedetomidine infusion on postoperative shivering and the change of the body temperature during general anesthesia and epidural ansthesia. Patients were randomly divided into two goups; Group I (n=50) receiving general anesthesia and Group II (n=50) epidural anesthesia. After induction of general anesthesia Group I was divided into two subgroups. Similarly Group II was divided into two subgroups when the block reached to the T6 level. Group IA and Group IIA were administered saline solution, where Group IB (n=25) and IIB received 2 µg mL-1 prepared dexmedetomidine solution, 0.25 mL kg-1 hr-1 dexmedetomidine infusion was administered for 10 minutes subsequently 0.1 mL kg-1 hr-1 infusion for maintenance. The heart rate (HR), systolic arteriel pressure (SAP), diastolic arteriel pressure (DAP), peripheral oxygen saturation (SpO2) tympanic membrane temperature of all patients were recorded after induction or block and at 30, 60, 90th minutes. In Group II sedation scores were also recorded. HR, SAP, DAP, SpO2 tympanic membrane temperature, shivering scores, sedation scores were recorded post operative at 15, 30, 60th minutes. The patients who had shivering score more than 3 were administered intravenous 25 mg meperidin. The dosage and times of meperidin given were recorded. Postoperative 60th minute the patients who have Aldtrete score of 9 were externed to their services. Anestezi sonrası titreme tedavisinde farmakolojik ajanlar tercih edilmektedir. İdeal ve yan etkisi az efektif bir ajan bulmak için çalışmalar devam etmektedir. Çalışmamızda, genel ve epidural anestezi uygulanan olgularda deksmedetomidin infüzyonunun operasyon sırasında ve sonrasındaki ısı değişikliği ile anestezi sonrası titreme üzerine etkisini araştırmayı amaçladık.Olgular rasgele Grup I (n=50) genel anestezi; Grup II (n=50) epidural anestezi olacak şekilde ayrıldı. Grup I indüksiyondan, Grup II blok T6'ya ulaşınca, kendi içinde 2'ye ayrılarak, Grup IA (n=25) ve IIA (n=25)'ya serum fizyolojik ve Grup IB (n= 25) ve IIB'ye deksmedetomidin 2 µg mL-1 olacak şekilde hazırlanmış solüsyon, ilk 10 dk 0.25 mL kg-1 sa-1, sonra 0.1 mL kg-1 sa-1 infüzyon uygulandı. Postoperatif 15., 30. ve 60. dk.'da KAH, SAB, DAB, SpO2, timpanik membran ısısı, titreme skoru (TS) ve SS değerleri kaydedildi. TS e3 olan olgulara 25 mg meperidin iv. yapılarak dozu ve sayısı kaydedildi. Postoperatif 60 dk. Aldrete derlenme skoru 9 olan olgular servise çıkarıldı. İntraoperatif 30. ve 90. dk.'da KAH ve DAB değerleri; 30., 60. ve 90. dk.'da timpanik membran ısısı değerleri Grup IA'da, Grup IB'ye göre düşük bulundu (p
