Tetanus in our intensive care unit:Review of therapy, complications and mortality in seventeen patients

Abstract

Tetanus is an acutely progressive, morbid disease that results from exposure to the exotoxins produced by Clostridium tetani. The disease has an importance because of its high mortality especially for the patients older than 60 years. Respiratory failure is the most severe problem related to generalized tetanus. Mechanical ventilation is almost mandatory for the therapy of respiratory insufficiency and airway obstruction developed in the patients with tetanus. The purpose of the study was to determine the frequency of tetanus and to evaluate the patients with tetanus admitted to the Anaesthesiology intensive care unit retrospectively, in terms of the route of infection, immunization, clinical findings, management, duration of mechanical ventilation, complications, intensive care unit stay and mortality. Between the years 1997 and 2006, overall 17 patients were admitted to Akdeniz University Medical Faculty Anesthesiology and Intensive Care Unit (ICU) department with the diagnosis of tetanus. Twenty percent of the patients with tetanus were older than 60 years. Incidence of mechanical ventilation was 77 % for these patients. Percutaneous dilatational tracheostomy was performed at all of the mechanically ventilated patients with tetanus. The mean duration of mechanical ventilation was 23.2±6.7 days (14-34 days). Mean ICU stay was 24±12.3 days (5-48 days). The most frequent complication was pneumonia ventilator associated pneumonia. Overall mortality for the tetanus patients was % 17. Supportive therapies have great importance besides the specific medical therapies of tetanus. Mean duration of mechanical ventilation is approximately 1 month for the patients. Complications related to the main pathology or ongoing therapies may develop during this period as we reported in this study. For the purpose, patients with tetanus should be treated in the modern intensive care facilities where mechanical ventilation support is available and complications are well handled

Tetanoz akut gelişen, ölümeül seyredebilen, Clostridium tetani'nin ürettiği ekzotoksinlerin neden olduğu bir hastalıktır. Özellikle 60 yaş üzerinde yüksek mortalitesi nedeniyle önemlidir. Solunumsal tutulum jeneralize tetanozda görülen en ciddi sorundur. Hastalarda görülen solunum yetersizliği ve havayolu obstrüksiyonu sıklıkla ventilator desteğini gerektirmektedir. Bu çalışmanın amacı, anestezi yoğun bakım ünitesinde tetanoz sıklığı ve izlediğimiz tetanozlu hastaların bulaş yolu, immünizasyon durumu, klinik bulguları, uygulanan tedaviler, mekanik ventilasyon süresi, ortaya çıkan komplikasyonlar, yoğun bakımda kalış süresi ve mortalite açılarından retrospektif olarak değerlendirilmesidir. 1997-2006 yılları arasında Akdeniz Üniversitesi Tıp Fakültesi Anesteziyoloji ve Reanimasyon Yoğun Bakım Servisi'nde toplam 17 tetanoz olgusu takip edilmiştir. Hastaların % 20'si 60 yaş üzerinde olduğu belirlenmiştir. Olgularımızın % 77'sine mekanik ventilasyon uygulanmıştır. Mekanik ventilatörde izlenen tüm olgularımıza trakeostomi açılmıştır. Hastaların mekanik ventilatörde izlenme süreleri ortalama 23.2±6.7 gün (14-34 gün) olarak saptanmıştır. Hastaların yoğun bakımda kalış sürelerinin ise, ortalanma 24± 12/3 gün (5-48 gün) olduğu tespit edilmiştir. En sık gözlenen komplikasyon ventilator ilişkili pnomonidir. Mortalitemiz % 17 olarak belirlenmiştir. Tetanoza yönelik medikal tedavinin yanı sıra destek tedavileri de büyük önem taşımaktadır. Hastaların ortalama mekanik ventilasyon süreleri yaklaşık 1 aydır. Bu süre içinde gerek altta yatan asıl patolojiye gerekse tedavilere bağlı olarak solunumsal, enfeksiyöz veya kardiyak birçok sorunların ortaya çıktığı çalışmamızda da görülmüştür. Bu nedenle tetanoz tedavisi, mekanik ventilasyon desteğinin sağlanabildiği ve izlem sırasında gelişebilecek sorunlarla yeterli mücadelenin uygulanabileceği modern yoğun bakım ünitelerinde yapılmalıdır.
