One-lung ventilation in a patient with anxylosing spondylitis

Abstract
Ankylosing spondylitis is a progressive inflammatory disease characterized by stiffening of the joints. Stiffness of the cervical spine, atlantooccipital and temporomandibular joints might lead to difficult intubation. One-lung ventilation is the most important component of an anesthetic management in patients undergoing thoracic surgery. Usage of double-lumen tube is the method of choice for one-lung ventilation in most of these patients. When double-lumen tube placement fails or inappropriate, Univent® bronchial blocker tube may be required. In this report, we wanted to emphasize the usage of Univent® bronchial blocker tube for one-lung ventilation in a patient suffering from ankylosing spondylitis. In this patient difficult intubation and requirement for postoperative mechanical ventilation were anticipated.

Ankilozan spondilit, eklemlerde hareket kısıtlılığı ile karakterize, ilerleyici inflamatuvar bir hastalıktır. Servikal vertebral kolon, atlantooksipital ve temporomandibular eklem tutulumu entübasyon güçlüğüne yol açabilir. Tek akciğer ventilasyonu (TAV), göğüs cerrahisi uygulanan hastalarda anestezi uygulamasının çok önemli bir unsurudur. Bu hastaların çoğunda çift lümenli tüp kullanılması seçilen TAV yöntemidir. Çift lümenli tüp yerleştirmenin zor veya uygunsuz olduğu durumlarda, Univent® bronşiyal bloker tüpe gereksinim olabilir.Bu yazımızda; entübasyon zorluğu beklediğimiz ve postoperatif dönemde mekanik ventilasyon gereksinimi olabilecek bir ankilozan spondilit olgusunda, TAV için Univent® bronşiyal bloker tüp kullanımı deneyimimizi paylaşmak istedik.
