A pharmacogenetic approach to warfarin sensitivity

Abstract
In determining the optimal dosage of Warfarin, defining the CYP2C9 genotype (cytochrome p450 CYPC2C9) which regulates the metabolism of the medicine has become very important as well as age, sex, weight and interaction with other drugs. We report the case of this patient who had an operation of an ovary cyst rupture and had developed during the post-operative period a widespread echimosis; treated on warfarin, we found out that this patient was carrying the mutant allel CYP2C9*3. We believe that the CYP2C9* 1 *3 mutant genotype may contributeto the development of the intra-abdominal hemorrhage and a widespread echimosis; we also wanted to point the fact that defining the CYP2C9 genotype before to start when it's necessary the Warfarin treatment, might be useful.
Özet
Warfarinin optimal dozunun belirlenmesinde yaş, cins, kilo ve birlikte kullanılan diğer ilaçlarla etkileşiminin yanısıra ilacın metabolizmasını düzenleyen CYP2C9 (Sitokrom P450 CYP2C9) genotipinin belirlenmesi de son yıllarda büyük önem kazanmıştır. Biz de over kist rüptürü nedeni ile öpere edilmiş ve postoperatif dönemde yaygın ekimoz gelişmiş Warfarin kullanan olgumuzun CYP2C9*3 mutant genotipini heterozigot olarak taşıdığını saptadık. Olgumuzdaki intraabdominal kanama ve yaygın ekimozun gelişmesinde CYP2C9* 1*3 mutant genotipinin etyolojik faktör olarak rol oynayabileceği ve Warfarin tedavisi alması planlanan hastalarda tedavi öncesinde CYP2C9 genotipinin belirlenmesinin faydalı olabileceğini vurgulamak istedik.
