A pulmonary nocardiosis in a patient who had had liver cirhosis and renal transplantation: Case report

Abstract
Opportunistic infections are more frequent in transplanted and immunosuppressed patients. This situation is also troublesome in making the right diagnosis and treatment. The infections after organ transplantation are classified according to whether they develop in the first month or between 1-6 months and beyond 6 months after the transplantation. Beyond 6 months after the transplantation, infections which are characteristic of patients with defective cell-mediated immunity-e.g., infections with Listeria monocytogenes, nocardia, various fungi, and other intracellular parasites-may be encountered. Nocardiosis is one of the member of these opportunistic infections. It is typically subacute but rarely can be acute. It can be seen in different clinical forms which are pulmonary dissease, brain abcess, skin involvement, systemic nocardiosis and actinomycetoma. In this article, we report a pulmonary nocardiosis with cavitation in a patient who had had liver cirhosis and renal transplantation.

Organ transplantasyonu sonrasında enfeksiyonlar, transplantasyon sonrası 1. ay, 1-6. ay, 6. ay ve daha sonrası dönemde ortaya çıkmasına göre sınıflandırılmaktadır. Transplantasyonun 6. ay ve daha sonrasında fırsatçı infeksiyonlar (Pneumocytsis carinii, Listeria monocytogenes, funguslar, nokardia ve diğer intraselüler parazitler) sorun olabilmektedir. Bu fırsatçı infeksiyonlardan birisi de Nokardiyozis olup nadiren akut ancak genelde subakut seyreder. İnsanlarda akciğer, santral sinir sistemi, deri tutulumu, sistemik nokardiyoz, ve aktinomiçetoma olmak üzere değişik klinik formlarda görülebilir. Burada hepatit C virüsüne bağlı dekompanse karaciğer sirozu olan renal transplantlı bir hastada kavitasyonla seyreden pulmoner nokardiyozis olgusu sunulmaktadır.
