The predictive significance of lymphocyte subgroups in the diagnosis of tuberculosis: Comparison with other methods

Abstract
We planned to compare the conventional diagnostic methods such as Polymerase Chain Reaction (PCR), Bactec, Lowenstein-Jensen (LJ) and detection of acid resistant bacilli (ARB) in direct smear preparations with the value of lymphocyte subgroups in sputum and blood samples from 44 patients with acute pulmonary tuberculosis and 40 asymptomatic cases. Mvcobacterium tuberculosis was determined in 41 of 44 (93.2 %) sputum samples by PCR, in 37 of 44 (84.1 %) samples by Bactec,. in 31 of 44 (80.4 %) samples by LJ and in 29 of 44 (65.9 %) samples by direct staining in the patient group. When PCR assay was refered as a reference test, the value of sensitivity and specifity of Bactec, LJ, direct staining were determined as 90.2 % and 100 %, 75.6 % and 100 %, 70.7 % and 100 %, respectively. CD2(111), CD4 (14), and CD8 (18) did not show any statistical difference in the distribution of lymphocytic subgroups, butCD20 (B1) and NK cells did. Although lymphocyte subsets have a main protective role in patients with active pulmonary tuberculosis, limited data on their diagnostic value in peripheral blood samples of patients with tuberculosis have been obtained in this study.

Akut tüberküloz tanısında Polimeraz Zincir Reaksiyonu (PCR), Bactec, Lowenstein-Jensen (LJ), direkt yayma preparatlarda asido-rezistan basil (ARB) aranması gibi konvansiyonel tanı yöntemleri ve lenfosit subgruplannın tanı değerini karşılaştırmak amacı ile planlanan çalışmada akciğer tüberküloz tanılı 44 hasta ile asemptomatik 40 kişiye ait balgam ve kan örnekleri değerlendirilmiştir. Hasta grubunda 41 (% 93.2) balgam örneğinde PCR ile, 37 (% 84.1) örnekte Bactec ile, 31 (% 70.4) örnekte LJ ve 29 (% 65.9) örnekte boyalı preparasyonlarda bakteri varlığı saptanmıştır. PCR testi referans olarak alındığında Bactec, LJ, direkt yaymanın duyarlılık ve özgüllük oranlarının sırası ile % 90.2 ve % 100, % 75.6 ve 100, % 70.7 ve % 100 olduğu görülmüştür. Her iki grupta yer alan kişilerde lenfosit subgruplannın dağılımında CD20 (B1) ve NK hücreleri dışında CD2 (111), CD4 (14), CD8 (18) 'de istatistiksel bir fark görülmemiştir. Akut pulmoner tüberkülozlu hastalarda lenfosit subgruplannın önemli koruyucu rolü olmasına rağmen, bu çalışmada tüberkülozlu hastalarda lenfosit subgruplannın tanısal değeri ile ilgili sınırlı veri elde edilmiştir.
