The comparison of ELISA IgG levels with immunochromatographic test (ICT) in patients with tuberculosis

Abstract
The diagnosis of infection caused by Mycobacterium tuberculosis is of increased public health concern following recent increases in the number of cases in developing countries. A rapid membrane based antibody assay capable of diagnosing pulmonary tuberculosis within 15 minutes has been developed using 38kDa antigen from Mycobacterium tuberculosis. The aim of this study was to determine the specificity and sensitivity of this assay and evaluate its usefulness in a clinical setting. Fourteen patients with high specific IgG levels to all three antigens (BCG, PPD, sonicate) by ELISA and 11 healthy controls were compared by rapid immunochromatographic test (ICT). Rapid test (ICT) was found to be 96 % in accordance with ELISA IgG levels. It can be concluded that ICT can be used in the rapid diagnosis of tuberculosis in wide populations as a supplementary test. To be used routinely it needs further investigations in populations having high tuberculosis prevalence and in more cases having borderline IgG levels in ELISA.

Mycobacterium tuberculosis 'in hızlı tanısı, son zamanlarda gelişmekte olan ülkelerde hastalığın artışı nedeniyle giderek bir halk sağlığı sorunu haline gelmiştir. Son zamanlarda tüberkülozun 38kDa antijeni ile 15 dakikada akciğer tüberkülozunun tanısında sonuç veren kard test geliştirilmiştir. Bu çalışmada kard testin duyarlılığı ve özgüllüğü enzim linked immün assay (ELISA) sonuçlan ile birlikte değerlendirilerek klinik uygulamada yeri tartışılmıştır. ELİSA'da üç antijenle (BCG, PPD, sonikat) yüksek düzeylerde IgG yanıtları alınmış olan 14 hasta ile 11 sağlıklı kontrol serumları immunokromatografik (ICT) yöntemle çalışılmıştır, iki test arasında genel uyumluluk %96 olarak bulunmuştur. Bu testin geniş popülasyonlan tarama testi olarak kullanılabileceğinin, ancak Türkiye gibi hastalığın prevelansının yüksek olduğu toplumlarda, rutin çalışmalarda kullanılabilmesi için daha çok sayıda ve sınırda olgular ile çalışılması gerekir.
