Significance of the isolation of Candida species from endotracheal aspirates in mechanically ventilated intensive care patients

Abstract
The diagnosis of pulmonary candidosis is still controversial. In this prospective study, 50 mechanically ventilated (72 h) in the Intensive Care Unit (ICU) were studied with the aim of assessing the incidence and significance of the isolation of Candida species from endotracheal aspirates. Patients were categorized as individuals already colonized with Candida spp. on hospitalization in the first, individuals colonized during hospitalization in the second and patients with no colonization in the third group. The percentage of patients already colonized with Candida was low (6 of 50; 12 %), the incidence of Candida isolation from endotracheal aspirate specimens in critically ill, mechanically ventilated patients in ICU was also low (6 of 50; 12 %), and possible Candida pneumonia was established in one of 50 (2%) patients. Coincidence of Candida colonization with possible Candida pneumonia was quite low (1 of 12; 8.3%); that is, the presence of Candida in lower respiratory samples was not a good marker of Candida pneumonia in critically ill patients. Age, duration of hospitalization, pre-treatment with antimicrobials or immunosupressive agents and occurrence of underlying disease were found to be not risk factors. However, both antifungal usage and neutropenia incidence were higher in already colonized patients and these two factors seemed to increase the risk in already colonized individuals. On the other hand, there was no risk factor determined in patients colonized during hospitalization in ICU. As all the isolates identified were C. albicans. It appears that at least for now, colonization and/or infection by more resistant Candida species is not a problem for ICU in the authors' hospital.

Pulmoner kandidoz tanısı tartışmalı bir konudur. Bu nedenle, bu prospektif çalışmada Reanimasyon Ünitesi (RÜ)'nde üç günün üzerinde izlenen ve en az 72 saat mekanik olarak ventile edilen 50 hastanın endotrakeal aspiratında Candida türleri izolasyonu sıklığı ve bunun klinik önemi araştırılmıştır. Hastalar; RÜ'ne alındıklarında Candida türleri ile kolonize olanlar, ünitede kolonizasyon gelişenler ve Candida ile hiç kolonize olmayanlar olmak üzere üç grupta değerlendirildi. Üniteye Candida ile kolonize durumda gelen hastaların oranı oldukça düşük (50 hastadan altısı; %12) bulundu; altı hasta ise ünitede kolonize oldu. Çalışma süresince Candida pnömonisi düşünülen tek bir hasta olması yani Candida kolonizasyonu ve muhtemel Candida pnömonisi birlikteliğinin 12 hastadan sadece birinde (%8.3) saptanması alt solunum yollarından Candida izolasyonunun pnömoni tanısına yardımcı olacak bir bulgu olmadığını düşündürmektedir. Candida kolonizasyonu açısından ayrılan üç hasta grubunda yaş, hospitalizasyon süresi, önceden antibiyotik ve immunosupresif ajan kullanımı ve eşlik eden hastalıkların varlığı gibi faktörler bakımından fark saptanmazken antifungal kullanımı ve nötropeni insidansı üniteye Candida ile kolonize olarak gelen hastalarda anlamlı olarak yüksek bulundu. Ünitede kolonize olan hastalarda ise herhangi bir risk faktörü tanımlanamadı. İzole edilen tüm Candida izolatlarının C. albicans olması RÜ'de antifungallere daha dirençli olabilen türlerle kolonizasyon/infeksiyonun en azından şimdilik sorun olmadığını göstermektedir.
