Comparison of methods used in the diagnosis of intestinal parasites

Abstract
The evaluation of stool for parasites involves the use of various methods. The purpose of this study was to compare the different diagnostic methods for intestinal parasites. Six hundred fourty-one students of a primary school in the center of Aydın were included in the study. The study population ranged in age from 6 to 10 years. Parasitologic diagnosis was made with direct wet mount, formol-ethyl acetate sedimantion technique, trichrome stain and cellophane tape method in the Department of Microbiology and Clinical Microbiology, Medical Faculty, Adnan Menderes University. One hundred thirty-four (20.9%) students were infected by intestinal parasites: 85 (13.3%) had Enterobius vermicularis, 29 (4.5%) Giardia intestinalis, 15 (2.3%) Blastocystis hominis, 6 (0.9%)Entamoeba coli, 1 (0.2%) lodomoeba butschlii and 1 (0.2%) Hymenolepis nana. Three of children had double parasites identified in the single stool specimen. In all 85 cases who had Enterobius vermicularis, the parasite was found with cellophane tape method. Although the parasite was found in trichrome stain in all 29 cases who had Giardia intestinalis, 25 of them were found in sedimentation technique and 14 of them were found in direct wet mount. Fifteen cases of Blastocystis hominis and one case of /. butschlii were detected in only trichrome stained smear. Although in all 6 cases who had Entamoeba coli, parasites were found in sedimentation technique and trichrome stain, 2 of them were found in direct wet mount method. In one specimen H. nana was detected in both with direct wet mount and formol-ethyl acetate sedimentation technique but was not detected in trichrome stained smear. In conclusion, it is not sufficient to investigate the faeces by only one method to diagnose parasite infections. Particulary cellophane tape method should be used to diagnose E. vermicularis and sedimentation technique and trichrome stain methods to diagnose other parasites.

Bağırsak parazitlerinin laboratuvar tanısında farklı yöntemler kullanılabilmektedir. Bu çalışmada bağırsak parazitlerinin tanısında uygulanan yöntemlerin karşılaştırılması amaçlanmıştır. Aydın ili merkezinde bir ilköğretim okulunda öğrenim gören 6-10 yaş grubundaki 641 öğrenci çalışma kapsamına alınmıştır. Dışkı örneklerinde parazitoiojik inceleme Adnan Menderes Üniversitesi Tıp Fakültesi Mikrobiyoloji ve Klinik Mikrobiyoloji Anabilim Dalı Laboratuvarı'nda direkt bakı, fprmol-etii asetat çöktürme, trikrom boyama ve selofan bant yöntemleri ile yapılmıştır. Toplam 641 çocuğun 134 (%20.9)'ünde bağırsak paraziti saptanmıştır. Çocukların üçünde iki ayrı cins parazit bulunmuştur. Taranan öğrencilerin 85 (%13.3)'inde Enterobius vermicularis, 29 (%4.5)'unda Giardia intestinalis, 15 (%2.3)'inde Blastocytis hominis, altısında (%0.9) Entamoeba coli, birinde (%0.2) Hymenolepis nana, birinde (%0.2) lodamoeba butschlii saptanmıştır. Enterobius vermicularis saptanan 85 olgunun tamamında parazit selofan bant yöntemi ile saptanmıştır. Giardia intestinalis bulunan 29 olgunun tümünde parazit trikrom boyamada saptanırken, çoklaştırma ile 25'i direkt bakı ile 14'ü saptanabilmiştir. Blastocystis hominis saptanan 15 olgunun tamamında ve /. butschlii bulunan bir olguda parazit sadece trikrom yöntemi ile saptanmıştır. Entamoeba coli saptanan altı olgunun tümünde çoklaştırma ve trikrom yöntemi ile parazit bulunurken direkt bakı ile ikisi saptanabilmiştir. Hymenolepis nana bir olguda hem direkt bakı hem de çoklaştırma yöntemi ile saptanırken trikrom yöntemi ile saptanamamıştır. Bağırsak paraziti infeksiyonların tanısında dışkının tek yöntem ile incelenmesinin yeterli olmadığı, özellikle E. vermicularis yönünden kesinlikle selofan bant yönteminin, diğer parazit infeksiyonlarının tanısında ise çoklaştırma ve trikrom boyama yöntemlerinin kullanılmasının uygun olacağı düşünülmektedir.
