Reconstruction of columella and septal defect resulted from gunshot injury with supratrochlear flap: A case report

Abstract

The columella is an important aesthetic ıınit in the midface and its absence may cause signiflcant functional and aesthetic problems. Reconstruction of this region presents challenging problems. Supratrochlear flap, a modification of paramedian flap was used for the reconstruction of both columella and septum. During reconstruction functional competence as well as cosmetic appearance must be achieved. Local and distant flaps are necessary to reach this purpose. Here in this case, Supratrochlear flap, a modification of paramedian flap, provided a certain chance to reconstruct both columella and septum together where reconslructive options are limited due to considerable injury and scarred tissue around. Supratrochlear flap may be considered as öne of the options for reconstruction ofthis anatomic region.

Özet

Kolumella, orta yüzde önemli bir ünite olup eksikliğinde ciddi fonksiyonel ve estetik sorunlar oluşmaktadır. Kolumella ve kaudal septum onarımında fonksiyonel bütünlük kadar kabul edilebilir kozmetik görünümün de kazandırılması önemlidir. Bu amaçla kolumella onarımı için çeşitli lokal ve uzakflepler tanımlanmıştır. Onarım seçeneklerinden bir tanesi de paramedian flebin bir modifikasyonu olan supratroklear fleptir. Bu makalede intihar girişimine bağlı maksillofasyal ateşli silah yaralanması sonrasında kolumella ve kaudal septum onarımı için supratroklear flep kullanımı ve sonuçları sunulmaktadır. Çevre dokuların skarlı ve yaralanmış olması nedeni ile fazla onarım alternatifinin olmadığı olgumuzda supratroklear flep ile onarım iyi bir alternatif olarak görülmektedir.
