The role and criterias of neck dissection for the treatment of epidermoid carcinoma of the head and neck region: 5 years experience

Abstract

In the treatment of the head and neck epidermoid carcinomas, the ultimate goal is destroying the primary focus, local and distant metastases. Neck dissections are used as surgical method to remove local metastasis. in our clinic, neck dissection was applied to the 77 patients with head and neck epidermoid carcinomas in addition to excision of the primary tümör between the dates of July 1996 and July 2001. Regarding to the preoperative staging and postoperative pathologic diagnosis of the patients, it has been detected that the ratio of metastating to the cervical lymph nodes increases as the primary tümör dimension expands and the differantiation degree diminishes. Among the 54 patients suffering from lower lip tümör, mostly the cervical metastases had taken place in the group of commissurally located tumors. The recurrence rate was higher in the patients having cervical metastases; additionally their survival period for threeyears vcere lower when comparedvvith the group having no cervical metastases. Besides, lymph node metastase was detected in 25 % of patients, who had not acquiredany cervical lymphadenopathy in their preoperative period and undergone cervical dissection regarding the other criteria (such as tümör dimension, differantiation degree, location in the lower lip tumors, adaptation to the long-lasting therapy of the patient). The most frequently encountered complication was observed as suture dehiscence.

Özet

Baş-boyun bölgesinde yerleşen epidermoid kanserlerin tedavisinde amaç primer odağın ve varsa lokal ve uzak metastazların ortadan kaldırılmasıdır. Lokal metastazların giderilmesinde cerrahi yöntem olarak boyun diseksiyonlan kullanılmaktadır. Kliniğimizde temmuz ~1996-Temmuz 2001 tarihleri arasındaki 5 yıllık periyodda baş-boyun yerleşimli epidermoid kanserlerin tedavisi amacıyla 77 hastaya primer tümör eksizyonu yanında boyun diseksiyonu uygulanmıştır. Hastaların preoperatif dönemdeki evrelemeleri ve postoperatif patolojik tanılarına bakıldığında primer tümörün boyutu arttıkça, diferansiasyon derecesi azaldıkça boyun lenf bezlerine metastaz yapma oranının arttığı görülmüştür. Alt dudak tümörü olan 54 hastada boyna en çok metastaz komissur yerleşimli tümörlerde olmuştur. Boyunda metastaz olan hastalarda nüks daha yüksek oranda görülmüş ve bu hastaların 3 yıllık yaşam oranları boyun metastazı olmayanlardan daha düşük olmuştur. Ayrıca boynunda preoperatif dönemde lenfadenopati (LAP) saptanmayan, diğer kriterler (tümör boyutu, diferansiasyon derecesi, alt dudak tümörlerinde tümörün yeri, hastanın uzun süreli tedaviye uyumu) gözönüne alınarak boyun diseksiyonu yapılan hastaların %25 'inde lenf bezi metastazı bulunmuştur. En sık rastlanan komplikasyon sütür ayrılması olarak belirlenmiştir.
