Reconstruction of sacral soft tissue defects using pedicled superior gluteal artery perforator flap

Abstract
Various flaps with musculocutaneous and fasciocutaneous features were used in teratment of sacral tissue defects. In this study the advantages of superior gluteal artery perforator (SGAP) flap, which is one of the several popularized perforator flaps, for the reconstruction of this region is discussed. Between -2002 and 2004, 8 patients with sacral tissue defects were treated using this technique. Diameters of the defects ranged between 12x9 and 30x42 cm. Nine flaps were used that one of the patients necessitated bilateral flap. Seven flaps survived and healed uneventfully, however 2 flaps were lost due to total necrosis. No early and late complications were evident, except postoperative hematoma was seen under the flap in one patient. Mean hospitalization period was 7 days. No recurrence was seen during 11 months follow-up. In this technique, early and late complication rates are lower. Early mobilization of the ambulatory patients and prevention of muscle weakness in the future is provided due tö preservation of gluteal muscle. SGAP flap, which is raised on a single perforator vessel, owns the same amount of blood pefusion as musculocutaneous flaps, and serves a reliable alternative method for the reconstruction of sacral tissue defects.
Özet
Sakral doku defektlerinin tedavisinde muskulokütan ve fasyokütan özellikte çeşitli flepler kullanılmıştır. Bu çalışmada, son zamanlarda popülaritesi artan perforator fleplerden biri olan superior gluteal arter perforator (SGAP) flebin bu bölgenin rekonstniksiyonunda kullanımı tartışılmıştır. 2002-2004yılları arasında sakral doku defekti nedeniyle başvuran 8 hasta bu teknik kullanılarak ameliyat edildi. Sakral defektlerin boyutları 12x9 ile 30x42 cm arasında^ değişmekteydi. Bir hastada bilateral flep gerekmesi nedeniyle toplam 9 flep kullanıldı. Yedi flepte tam sağ kalım elde edilirken, 2 flepte total nekroz görüldü. 1 hastada gelişen postoperatif hematom dışında erken ve geç komplikasyon görülmedi. Ortalama hastanede kalış süresi 7 gündü. 11 ay süreyle takip edilen hastalarda nüks görülmedi. Bu teknikte, gluteal kasın korunması nedeniyle erken ve geç komplikasyonlar daha az görülmekte ve yürüyebilen hastalarda erken mobilizasyon sağlanırken geç dönemde kuvvet kaybı görülmemektedir. Tek bir perforator damar üzerinde kaldırılan ve kas-deri flepleriyle aynı miktarda dolaşıma sahip olan SGAP flep, sakral doku defektlerinin tedavisinde güvenle kullanılabilecek bir alternatiftir.
