The effect of bupivacaine or tenoxicam on postoperative pain and analgesic consumption fallowing flank incisions

Abstract
In this study, it was aimed to compare the efficacy of bupivacaine and tenoxicam infiltration for postoperative pain after renal surgery. Forty-five ASA I-II class 45 patients, scheduled for renal surgery was randomized into three groups. A standardized general anaesthetic protocol was used in all of the groups. Before wound closure groups received 20 mg tenoxicam in 30 mL % 0.9 NaCl and 30 mL 0.25 % bupivacaine and 30 mL % 0.9 NaCl infiltrations respectively. Systolic and diastolic arterial blood pressures and heart rate were recorded at 10 minute and at 1, 2, 4, 8, 12 ve 24 hours postoperatively. Visual Analog Scale was used to measure the pain at the same time interval. Postoperative analgesia was supplemented using patient controlled analgesia with meperidine. A record was made of total opioid consumption at 0-2, 2-4, 4-8, 8-12, 12-24 h intervals postoperatively. There wasn't any significant difference between groups regarding systolic, diastolic blood pressure and heart rate. The postoperative VAS pain scores were similar between the groups. Although the total consumption of meperidine in tenoxicam group was lower than bupivacain group and placebo (p>0.05). In conclusion, with the doses used, neither tenoxicam nor bupivacaine infiltration had no effect on postoperative pain and consumption of analgesic.

Çalışmamızda, böbrek ameliyatı yapılan olgularda insizyon bölgesine uygulanan tenoksikam ve bııpivakain infiltrasyonunun, postoperatif ağrı tedavisindeki etkinliklerini karşılaştırmayı amaçladık. Flank insizyonu ile böbrek ameliyatı planlanan, ASA l-II grubu 45 olgu çalışmaya alındı. Ameliyattan yarım saat önce 0.05 mg/kg İM midazolam ile premedike edilen olgular rasgele üç gruba ayrıldı. Bütün olgularda aynı genel anestezi yöntemi uygulandı. İnsizyon kapatılmadan önce Grup l (n=15) olgularda 30 mL serum fizyolojik içinde 20 mg tenoksikam, Grup ll (n=15) olgularda % 0.25'lik 30 mL bupivakain, Grup Ill (rı=15) olgularda ise 30 mL serum fizyolojik infiltrasyonu yapıldı. İndüksiyon öncesi, postoperatif 10. dk ile l, 2, 4, 8, 12 ve 24. saatlerde, olguların SAB, DAB, KAH değerleri kaydedildi. Postoperatif 10. ve 30. dk ile l, 2, 4, 8, 12, 14. saatlerde, VAS ile olguların ağrıları değerlendirildi. Postoperatif analjezi için hasla kontrollü analjezi kullanıldı ve 0-2, 2-4, 4-8, 8-12, 12-24. saatlerde kullanılan analjezik miktarı kaydedildi. Postoperatif SAB, DAB ve KAH bakımından da fark belirlenemedi (p>0.05). Ağrı skorları benzerdi. Postoperatif tüketilen meperidin miktarı tenoksikam grubunda, bupivakain ve kontrol grubuna göre daha az bulundu (p>0.05). Sonuç olarak; insizyon bölgesine infiltre edilen bupivakain ve tenoksikam kullandığımız dozlarda, flank insizyonuna bağlı postoperatif ağrı ve analjezik tüketimi üzerine etkili bulunmamıştır.
