Congenital pseudoarthrosis of clavicula

Abstract

Right clavicula fracture was found in a male patient who was brought to our outpatient clinic at the age of 7 days with a complaint of swel- ling on the right clavicula. The patient had no history of difficult de- livery or resuscitation. He was born at term by spontaneous vaginal delivery with a birth weight of 3 340 g from the first pregnancy of his mother and there was no consanguinity between the parents. Ortho- paedic examination repeated at the age of 2 months revealed patho- logical movement in the middle of the clavicula and a swelling with a diameter of 1 cm. There was no tenderness. Asymmetry was noted between the shoulders, but range of motion was normal in both shoulders. On radiographic examination, it was observed that the proximal and distal fragment in the middle region of the clavicula were displaced by 1 cm and there was no callus. It was noted that the medial end of the distal fragment was blunt and sclerotic. There was no other patho- logy related with the musculosketal system. Examination of the other systems was found to be normal. Thus, a diagnosis of congenital cla- vicula pseudoarthrosis was made. This condition is rare and there are less than 200 cases in the literature. Congenital clavicula pseudoart- hrosis almost always develops on the right side. Pseudoarthrosis was also on the right side in our patient. It is rarely bilateral (10%). It rarely occurs on the left clavicula and is usually associated with dextrocardia. Familial association is present in very few of the reported cases, but there is no genetic background. It is rarely diagnosed in the neonatal period. The diagnosis was made in the neonatal period in our patient. Shoulder functions are not affected, but painful deformity, cosmetic problems or thoracic outlet syndrome may be observed in the adoles- cence and afterwards. (Türk Ped Arş 2014; 49: 261-3)

Özet

Yedi günlükken sağ klavikula üzerinde şişlik yakınması ile çocuk po- likliniğimize getirilen erkek hastada sağ klavikula kırığı saptanmıştı. Hastada zor doğum ve yeniden canlandırma girişim öyküsü yoktu. Doğumda 3 340 g olan hasta zamanında normal kendiliğinden doğ- muştu. Hastanın annesinin ilk gebeliğiydi ve ebeveynleri arasında akrabalık yoktu. İki aylıkken tekrar yapılan ortopedik muayenede kla- vikula ortasında hareketli, 1 cm çapında şişlik vardı. Hassasiyet yok- tu. Omuzlar arasında asimetri dikkat çekiyordu. Ancak her iki omuz eklem hareket açıklığı normaldi. Çekilen grafide klavikula orta böl- gesinde proksimal ve distal parçanın 1 cm yer değiştirdiği, kallusun olmadığı görüldü. Distal parçanın mediyal ucunun künt ve sklerotik olduğu görüldü. Kas-iskelet sistemiyle ilgili başka patolojiyi düşün- dürecek bulguya rastlanmadı. Diğer sistem muayeneleri normal ola- rak değerlendirildi.Bunun üzerine doğuştan klavikula psödoartrozu tanısı konuldu. Bu durum nadir olup, literatürde 200den az olgu vardır. Doğuştan klavikula psödoartrozu hemen hemen daima sağda gelişir. Olgumuzda da psödoartroz sağdadır. Nadiren iki taraflıdır (%10). Sol klavikulada olması nadirdir ve genellikle kalp sağdadır. Bildirilen olguların çok azında ailesel birliktelik vardır; ancak genetik zemin yoktur. Yenidoğan döneminde tanı alması nadirdir. Olgumuz yenidoğan döneminde tanı almıştır. Omuz işlevleri etkilenmez; an- cak büyüme çağında veya sonrasında ağrılı şekil bozukluğu, kozme- tik sorunlar veya torasik çıkış sendromu görülebilir. (Türk Ped Arş 2014; 49: 261-3)
