Evaluation of children with lympadenopathy

Abstract
Aim: To examine children who present with enlargement of lymph nodes in terms of demographic, clinical, serological and radiological aspects. Material and Methods: Ninety-eight patients who presented withcomplaint of enlargement of lymph nodes were examined in termsdemographic, clinical, serological and radiological aspects by screening file data retrospectively. The character of lymph nodes (reactive, malign) was evaluated according to the distribution, number, sizes and blood supply determined in ultrasonographic measurements. Fisher s Exact test and Mann-Whitney U Test were used in comparison of the groups. Kappa value was used in assessment of compatibility between the two groups. Results: Cervical lymphadenomegaly was found most frequently in accor- dance with the complaint of swelling in the neck. Erythrocyte sedimenta- tion rate, ultrasonography, Epstein-Barr virus (EBV) and cytomegalovirus (CMV) IgM were not found to be statistically significatly different betwe- en the normal and abnormal physical examination groups (erythrocyte sedimentation rate; p=0.623, USG; p=0.753, EBV and CMV; p=1.00). Cyto- megalovirus and EBV IgM were not found to be statistically significatly different between the normal and abnormal ultrasonography groups (CMV; p=0.35, EBV; p=0.36). There was no compatibility between physical examination and ultrasonography (κ=0.32). Conclusion: Lymphadenopathy is a common problem in the childhood and necessitates a careful physical examination and follow-up. Labora- tory and imaging methods should be used when necessary. Although lymphadenopathy is mostly related with infections, care should be taken in terms of malignancy and malignancy should be eliminated. The im- portant point is systemic evaluation and follow-up of the patient. Itimportant to note physical examination findings and clinical follow-up findings, because frequent ultrasonographic investigations may confuse physicians and families with high values of lymph node measurements despite normal physical examination. Therefore, using our fingertips with a good physical examination is still our most sensitive diagnostic tool instead of ultrasonography. (Türk Ped Arş 2014; 49: 30-5)

Amaç: Lenf bezinde büyüme yakınması ile başvuran çocukları demogra- fik klinik, serolojik ve radyolojik yönden incelemektir. Gereç ve Yöntemler: Lenf bezlerinde büyüme yakınması ile başvuran 98 olgu geriye dönük olarak dosya bilgileri taranarak demografik klinik, serolojik ve radyolojik yönden incelendi. Lenf bezlerinin ultrasonogra- fi ölçümlerinde lenf bezlerinin dağılımı, sayısı, boyutları ve kanlanma durumuna göre yapısı (reaktif, malin) değerlendirildi. Grupların karşı- laştırılmasında Fishers Exact test ve Mann-Whitney U Test kullanıldı. İki grup arasında uyum değerlendirilmesinde kappa (κ) değerine bakıldı. Bulgular: En sık boyunda şişlik yakınması ile uyumlu olarak boyunda len- fadenomegali saptandı. Eritrosit çökme hızı, ultrasonografi, Epstein-Barr virüsü (EBV) ve sitomegalovirüs (CMV) İgM ile normal ve anormal fizik muayene grupları arasında istatistiksel anlamlı fark bulunamadı (eritro- sit çökme hızı; p=0,623, ultrasonografi; p=0,753, EBV ve CMV; p=1,00). Sitomegalovirüs ve Epstein-Barr virüs IgM ile normal ve anormal ultra- sonografi grupları arasında istatistiksel anlamlı fark bulunamadı (CMV; p=0,35, EBV; p=0,36). Fizik muayene ile ultrasonografi arasında uyum yoktu (κ=0,32). Çıkarımlar: Lenfadenopati çocukluk yaş çağında sık karşılaşılan bir so- run olup, dikkatli fizik muayene ve izlem yapılmalı, gerekli durumlarda laboratuvar ve görüntüleme yöntemlerinden faydalanılmalıdır. En sık enfeksiyonlara bağlı olsa bile malinite yönünden dikkatli olunmalı ve dışlanmalıdır. Önemli olan hastanın sistemik değerlendirilmesi ve izle- midir. Muayene bulgularının ve klinik izlemlerin kayda alınması önem- lidir. Çünkü sık yapılan ultrasonografik tetkikler fizik muayenede normal saptanmasına rağmen yüksek değerlerde lenf bezi ölçümleri ile aile ve doktorların kafasını karıştırabilmektedir. Bu yüzden ultrasonografi yeri- ne iyi bir fizik muayene ile parmak uçlarımızı kullanmak hala en hassas tanı aracımızdır. (Türk Ped Arş 2014; 49: 30-5)
