The diagnosis of thoracic malignant mesothelioma: Practical considerations and recent developments

Abstract

Pleural biopsies for the evaluation of malignant mesothelioma can besome of the most challenging cases faced by the practicing surgicalpathologist. We review the epidemiology, clinical presentation,and imaging studies in patients with malignant mesothelioma,and then present a practical approach to the diagnosis using thecytologic features for malignancy and whether there is an epithelioidor spindled morphology; four main scenarios are discussed. Thepertinent immunohistochemical work-up is reviewed for eachscenario. Following this general overview, more unusual histologicpatterns are compared and unusual presentations are discussed. Briefmention of grading systems for malignant mesothelioma and theuse of electron microscopy and molecular studies is made. Practicalconsiderations in the diagnosis of malignant mesothelioma are madethroughout.

Malign mezotelyomayı değerlendirmek için yapılan plevral biyopsiler, patoloji uzmanlarının karşılaştığı en karmaşık olgulardan birisi olabilir.
