Oral Kavite Kanserlerinde Sentinel Lenf Nodu Biyopsisinin Değeri

Abstract
Objective: The aim of this study was to establish theeffectiveness of sentinel lymph node biopsy in the de-tection of metastasis in N0 necks of T1-T2 early-stageoral cavity cancers.Methods: Twenty neck dissections were performed in18 patients diagnosed with T1 and T2 oral cavity cancer,with an indication for elective neck dissection betweenNovember 2007 and January 2011. The male to femaleratio was 12:8, with a mean age of 54.5 years (range 28-76). Eight of the dissections were performed for lower lip cancer, 7 for tongue cancer, and 5 for floor of themouth cancer. Sentinel lymph node biopsy was used todetect metastatic lymph nodes. Tc99m radionuclide in-jection was administered to the periphery of the tumor24 h before the operation, and a lymphoscintigraphy image was obtained 30 min after the injection. Amaç: Çalışmamızda T1-T2 erken evre oral kavitekanserinde N0 boyunlarda sentinel lenf nodu biyopsi-sinin metastazları saptamadaki etkinliğinin belirlenmesiamaçlanmıştır.Yöntemler: Kasım 2007-Ocak 2011 tarihleri arasındaT1-T2 oral kavite kanseri tanısıyla elektif boyun disek-siyonu endikasyonu konan 18 hastaya 20 boyun disek-siyonu uygulanmıştır. Çalışmaya katılan hastaların ka- dın erkek oranı 12:8 olup ortalama yaş 54.5 yıl (28-76) olarak bulunmuştur. Sekiz boyun diseksiyonu alt dudak kanseri için, 7si dil kanseri için, 5i ağız tabanı kanse-ri için uygulanmıştır. Metastatik lenf nodlarının tesbitiiçin sentinel lenf nodu biyopsisi yöntemi uygulanmıştır.Operasyondan 24 saat önce Tc99m içeren radyonüklidenjeksiyonu primer tümörün çevresine uygulanmış ve 30 dakika sonra lenfosintigrafi görüntüleri elde edilmiştir.Ameliyat günü statik lenfosintigrafi sonucu ve gammaprobu kullanılarak sentinel lenf nodları lokalize edile-a gamma probe. Sentinel lymph nodes were sent for afrozen section examination, and either a selective or acomprehensive neck dissection was performed for eachneck according to the results.
