The Effects of Superficial Musculoaponeurotic System Flap on the Development of Frey's Syndrome and Cosmetic Outcomes After Superficial Parotidectomy

Abstract
Objective: To investigate the outcomes of superficialmusculoaponeurotic system (SMAS) flap and classictechniques in superficial parotidectomy in terms ofFrey's syndrome (FS) and cosmetic satisfaction.Methods: In this study, a retrospective chart reviewof patients that underwent superficial parotidectomywas performed. These patients were divided into twosubgroups: group 1 included patients in which theSMAS flap was harvested and group 2 comprisedthe remaining patients on whom classic superficialparotidectomy was performed. All the patients wereevaluated clinically and with Minor's starch-iodinetest for FS. For the evaluation of the cosmetic results,the patient's satisfaction was queried according to theincision scar and surgical field skin retraction/facialsymmetry. Both groups were compared in terms ofcomplications and numbness of surgical area.Results: Fifty-five patients (31 male and 24 female)with a mean age of 50.19 years were included in thestudy. Thirty-two patients were in group 1 and 23 in ngroup 2. Thirteen patients (23.7%) described as havingFS and six of them were in group 1, while seven were ingroup 2. Minor's starch-iodine test was positive in ninepatients in group 1 (28.1%) and six patients in group 2(26.1%) (p=1.000). With regard to cosmetic satisfaction,eight patients (25%) stated mild discomfort from theincision scar and two patients (6.3%) stated cosmeticdissatisfaction for facial asymmetry in group 1. In group2 for the same factors the number of patients were 11(47.8%) and 2 two (8.7%), respectively (p=0.027). Therewere no statistically significant differences in means ofcomplication and numbness (p>0.05).Conclusion: According to our study results, there wasno superiority between both the groups in terms ofFS and incision scar satisfaction. We determined thatthere was a significant benefit of SMAS flap applicationin the prevention of volume loss and surgicalarea retraction.

Amaç: Süperfisyel muskuloaponörotik sistem (SMAS)flep ve klasik tekniklerle yapılan süperfisyel parotidektomininFrey Sendromu (FS) ve kozmetik memnuniyetaçısından sonuçlarını incelemek.Yöntemler: Bu çalışmada, süperfisyel parotidektomihastalarının kayıtları retrospektif olarak gözdengeçirildi. Bu hastalar iki alt gruba ayrıldı; grup 1'eSMAS'ın korunduğu hastalar alınırken, grup 2'yi geridekalan ve klasik süperfisyel parotidektomi uygulananhastalar oluşturdu. Tüm hastalar klinik olarakve Minor'ın nişasta-iyot testi ile FS için değerlendirildi.Kozmetik değerlendirme için insizyon skarıve cerrahi alan cilt retraksiyonu / fasial simetri ileilişkili hasta memnuniyeti sorgulandı. İkigrup arasında komplikasyon varlığı ve cerrahi alandauyuşukluk anlamında istatistiksel olarak anlamlı farklılıkyoktu (p>0.05).Sonuç: Çalışmamızın sonuçlarına göre FS ve insizyonskar memnuniyeti açısından iki grubun birbirine üstünlüğüyoktu. SMAS flep uygulamasının hacim kaybınıve cerrahi alan retraksiyonunu önlemede önemlibir yararı olduğu saptandı.
