Atypical Metastasis to the Head and Neck Region: An Analysis of 11 Patients

Abstract

Objective: We present 11 patients with distant metastases to the head and neck from an infraclavicularlylocated primary tumor and discuss the managementstrategies including the clinical presentation, treatment modalities, and prognosis.Methods: The retrospective data of the pathology reports and operation notes of 1239 patients who hadundergone any kind of oncological surgical intervention between 2005 and 2017 were analyzed. All of the11 patients included in the study were evaluated inour department’s tumor board, and all patients withan operable lesion had undergone surgery. Inoperablepatients were treated with chemotherapy and/or radiotherapy.Results: The average age of the patients was 64.3 (48–88) years. Primary tumors were located in the lung (2),breast (2), ovary (2), prostate (2), kidney (1), and colon(1) and the primary lesion could not be determined inone patient. The most common symptom was newlyoccurred painless swelling (9/11, 81.8%) at the metastatic site. Four patients without any other distantmetastases were operated. Of these four patients, twodied during follow-up due to systemic disease, and theother two are alive and disease-free. Three of the seveninoperable patients were treated with chemotherapyand the other four with radiotherapy. The prognosisof this group was worse.Conclusion: Although metastasis to the head andneck is not common, it is vital to keep in mind whileapproaching a patient with a lesion at the head andneck region especially if there is a history of lung, breast, and genitourinary cancers. Despite the poor prognosis, diminishing the tumor burden would increasethe treatment success.

Özet

Amaç: Bu çalışmada, infraklaviküler bölge yerleşim- li bir primer tümörden baş ve boyuna uzak metastaz yapmış 11 hasta ve bu hastaların klinik özellikleri, te- davi yöntemleri ve prognozlarını içeren yönetim stra- tejileri sunulmuştur. Yöntemler: 2005 ile 2017 yılları arasında herhangi bir nedenden dolayı onkolojik cerrahi girişim ya- pılmış 1239 hastanın dosyaları ve patoloji raporları geriye dönük olarak değerlendirildi. Çalışmaya dahil edilen 11 hastanın tamamı kliniğimizde gerçekleştiri- len onkoloji konseyinde değerlendirildi ve uygun olan tüm hastalara cerrahi tedavi uygulandı. Cerrahi yapı- lamayan hastalara ise kemoterapi ve/veya radyoterapi uygulandı. Bulgular: Hastaların yaş ortalaması 64.3 (48-88 yaş) idi. Primer tümörlerin yerleşim yeri akciğer (2), meme (2), over (2), prostat (2), böbrek (1) kolon (1) olarak saptandı ve bir hastada primer tümürün yerleşim yeri belirlenemedi. En sık görülen semptom metastaz bölgesindeki ağrısız şişlik idi (9/11, %81,8). Başka uzak metastazı olmayan dört hastaya cerrahi tedavi uygulandı. Bu dört hastadan ikisi, sistemik hastalık nedeniyle takipler sırasında kaybedildi, diğer ikisi ise hastalıksız olarak hayattadır. Cerrahi tedaviye uygun olmayan hastalardan dört tanesi radyoterapi, üç tanesi kemoterapi aldı. Bu grubun sağ kalımı çok daha kö- tüydü. Sonuç: Baş ve boyun bölgesine uzak metastaz sık gö- rülmemesine rağmen, özellikle akciğer, meme ve ge- nitoüriner sistem malignitesi öyküsü olan hastalarda baş ve boyun bölgesinde görülen bir lezyon varlığında metastaz olasılığı akılda bulundurmalıdır. Her ne ka- dar prognozu kötü olsa da tümör yükünün azaltılması tedaviye yanıt ve sağ kalımı arttıracaktır.
