Bone-cement ossiküloplasti: Uzun dönem sonuçlarımız

Abstract

Objective: Assessment of our long-term hearing resultsin a patient, in whom ossicle chain reconstruction wasestablished by using bone cement and conductive hear-ing loss was detected, was aimed.Methods: Files and surgery notes of twenty three pa-tients admitted to the Otorhinolaryngology Headand Neck Surgery Clinic of Şişli Etfal Training andResearch Hospital due to conductive hearing loss be-tween January 2006 and January 2010, and in whomossicle chain reconstruction with tympanoplasty and/or mastoidectomy was performed, were investigatedretrospectively. Tose admittes at the eighteenthmonths and later were included in the study. Te pa-tients being included into the study were studied interms of age, gender parameters, examination fn-dings and audiological examinations in the preoperative period, surgery notes and examination fndings in thepost-operative period, and audiological examinationsperformed after the eighteenth month.Results: In 21 (100%) of the 21 patients, graft membranewas intact. It was observed that ABG values fell below 20dB in seventeen patients (80.9%), below 15 dB in thirteen(61.9%) patients and below 10 dB in ten (47.6%) patients.In eighteen (85.7%) patients, improvement of more than10 dB was ensured compared to pre-operative ABG.Conclusion: Since favorable results of bone cementused for the purpose of ossicular reconstruction weredetected, this cheap and easily applicable material canbe used safety in ossiculoplasties.

Özet

Amaç: İletim tipi işitme kaybı saptanan ve bone cementkullanılarak kemikçik zincir rekonstriksiyonu yapılanhastalarda, uzun dönem işitme sonuçlarımızın değerlendirilmesi amaçlandı. Yöntemler: Ocak 2006 ile Ocak 2010 yılları arasında ŞişliEtfal Eğitim ve Araştırma Hastanesi Kulak Burun Boğazve Baş Boyun Cerrahisi Kliniğine iletim tipi işitme kaybınedeniyle başvuran ve timpanoplasti ve/veya mastoidektomi ile birlikte kemikçik zincir rekonstrüksiyonu yapılan,21 hastanın dosyaları ve ameliyat notları retrospektif olarak incelendi. Onsekizinci ay ve sonrasında kontrole gelmiş olanlar çalışmaya dahil edildi. Çalışmaya dahil edilenhastalar yaş, cinsiyet parametreleri, preoperatif dönemde- ki muayene bulguları, operasyon öncesi odyolojik tetkikleri, ameliyat notları ve postoperatif dönemdeki muayene bulguları, operasyon sonrası 18. aydan sonra yapılmış olanodyolojik tetkikleri incelendi.Bulgular: Hastaların tümünde greft zar intakttı. On yedihastada (%80,9) HKA değerlerinin 20 dBin altına, 13 hastada (%61,9) 15 dBin altına ve 10 hastada (%47,6) 10 dBinaltına düştüğü gözlenmiştir. 18 hastada (%85,7) ise preoperatif HKAna göre 10 dBden fazla iyileşme sağlanmıştır.Sonuç: Ossiküler rekonstriksiyon amacıyla kullanılan kemik çimentonun, işitme üzerine uzun dönemde de olumlu sonuçları saptandığından, bu ucuz ve kolay uygulanabilir materyal ossiküloplastilerde güvenle kullanılabilir.
