Inadequate Response to Osteoporosis Treatment

Abstract

It is well-known that effective treatment in patients with osteoporosis increases bone mineral density and decreases the risk of fractures. However, inadequate response to osteoporosis treatment may occur in some patients. Although, the concept of "inadequate response to treatment" is defined by considering the amount of the change in bone mineral density and/or the development of fractures in the relevant studies, no consensus on this issue has been reached yet. In this article, treatment approaches for osteoporosis, assessment of response to the treatment, the definition of inadequate response to osteoporosis treatment and approaches that should be applied in patients with inadequate response to treatment, were reviewed in the light of current literature.

Özet

Osteoporozu olan hastalarda etkin bir tedavi ile kemik mineral yoğunluğunda artış ve kırık riskinde azalma sağlandığı bilinmektedir. Bununla birlikte, bazı hastalarda osteoporoz tedavisine yetersiz yanıt meydana gelebilmektedir. Her ne kadar, bu konuda yapılan çalışmalarda "tedaviye yetersiz yanıt" kavramı, kemik mineral yoğunluğu değerlerindeki değişimin miktarı ve/veya kırık gelişimi göz önüne alınarak tanımlanmaya çalışılsa da, halen bu konuda tam bir fikir birliği oluşturulamamıştır. Bu yazıda; osteoporozda tedavi yaklaşımları, tedaviye yanıtın değerlendirilmesi, osteoporoz tedavisine yetersiz yanıt kavramı ve yetersiz yanıt olan hastalara uygulanması gereken yaklaşımlar güncel literatür eşliğinde gözden geçirilmiştir.
