The Association of ıntraocular pressure with vitamin d levels and bone mineral density in postmenopausal osteoporosis

Abstract

Objective: To investigate the effects of osteoporosis (OP) and vitamin D level on intraocular pressure (IOP) and to determine whether thoseconstitute a risk factor for glaucoma.Materials and Methods: Eighty postmenopausal patients with the diagnosis of OP (group 1) and 74 controls (group 2) were included inthe study. Age, height, weight and body mass index (BMI) of the patients were recorded. Bone mineral density (BMD) was measured fromthe lumbar vertebrae and proximal femur by using dual-energy x-ray absorptiometry (Lunar DPX-IQ®). The levels of 25(OH)-vitamin D weremeasured. Visual acuity was assessed by using Snellen charts. Gonioscopy was performed following the examination of the anterior segmentwith biomicroscopy. Applanation tonometry was used to measure the IOP at the same daytime. Dilated fundus examination was performedafter applying 1% tropicamide eye drops.Results: The mean age of the patients in group 1 and 2 were 62.4±10.5 and 60.6±11.9 years, respectively. Although the IOPs were higherin the group 1, the results were not statistically different (p>0.05). The difference between the levels of vitamin D were not statisticallysignifcant (p>0.05). There was a strong negative correlation between IOP and vitamin D (p=0.003, r=0.428). No correlations were foundbetween BMD, lumbar vertebral and femoral T-scores and IOP (p>0.05).Conclusions: The results of this study suggest that OP does not affect the IOP, but defciency of vitamin D may be a risk factor for higherIOPs. Thus, it can be recommended that vitamin D supplement may be useful in decreasing the higher IOP which is an important risk factorfor glaucoma. In the prevention of osteoporotic fractures related to falls routine ocular examination and measurement of IOP should beperformed. (Turkish Journal of Osteoporosis 2015;21: 1-4)

Amaç: Osteoporoz (OP) ve D vitamini düzeylerinin göz içi basınç (GİB) üzerine etkisini incelemek ve bunların glokom için bir risk faktörü olupolmadığını belirlemek.Gereç ve Yöntem: Osteoporoz tanısı konulmuş 80 postmenapozal kadın hasta (grup 1) ve 74 birey kontrol (grup 2) grubu olarak alındı.Hastaların yaş, boy ve kiloları sorgulandı, vücut kitle indeksi hesaplandı. Kemik mineral yoğunluğu (KMY) lomber vertebralardan ve proksimalfemurdan DXA cihazı (Lunar DPX-IQ®) ile değerlendirildi. 25(OH) D vitamin düzeyleri bakıldı. Her iki grupta Snellen eşeli ile görme keskinliğinebakıldı. Biomikroskop ile ön segment muayenesi ardından gonyoskopi yapıldı. Göz içi basınç aplanasyon tonometresi ile günün aynı saatindeölçüldü. Yüzde 1lik tropikamid damlatıldıktan sonra fundus muayenesi yapıldı.Bulgular: Grup 1 ve 2 hastaların yaş ortalamaları sırasıyla 62,4±10,5 ve 60,6±11,9 yıl idi. Her iki grup karşılaştırıldığında grup 1de GİB dahayüksek olmasına rağmen gruplar arasında istatiksel anlamlı fark yoktu (p>0,05). D vitamini düzeyi açısından gruplar arası istatiksel anlamlı farkyoktu (p>0,05). GİB ve D vitamini düzeyi arasında kuvvetli negatif korelasyon bulundu (p=0,003, r=0,428). KMY, lomber ve femur boyun Tskorları ile GİB arasında ilişki bulunamadı (p>0,05).Sonuç: Bu çalışma OPnin GİB düzeylerini etkilemediği, D vitamini eksikliğinin yüksek GİB gelişiminde risk faktörü olabileceğini göstermiştir.Biz glokom için en önemli risk faktörü olan GİB yüksekliğini azaltmada D vitamini takviyesinin faydalı olabileceğini düşünmekteyiz. Düşmelerebağlı osteoporotik kırıkları önlemede rutin göz muayenelerinin ve GİB ölçümlerinin yapılmasını önermekteyiz. (Türk Osteoporoz Dergisi2015;21: 1-4)
