Vitreoretinal surgery for retinal detachment with proliferative in vitreoretinopathy in eyes with lens dislocation

Abstract

Purpose : To evaluate the anatomic and visual outcomes of vitreoretinal surgery in the treatment of retinal detachment with proliferative vitreoretinopathy (PVR) in eyes with intravitreal lens dislocation. Methods : In this retrospective study. we examined 5 patients who had intravitreal lens dislocation and retinal detachment with PVR (trauma in 4 eyes and congenital glaucoma in 1 eye). Pars plana vitrectomy (PPV). removal of the dislocated lens and simultaneous reattachment of the retina by perfluorocarbon liquids (PFCL). removal of epiretinal membrane. if necessary retinectomy and removal of subretinal membrane; 360° scleral buckling. laser photocoagulation and silicon oil injection procedures were performed in these cases. Main outcome measures were anatomical and visual outcomes. Results : Mean age of the patients was 16.8 years (6-32 years) and mean follow-up time was 7 months (4-12 months). Retina was reattached totally in 3 cases. In one case. retinal redetachment occurred in the inferior quadrants without macular involvement at 4 months postoperatively. and remained stable during 12 months follow-up. Another eye. which was preoperatively prephytisic total retinal detachment with severe PVR developed at 1-week postoperatively. Preoperative visual acuity was light perception in all cases. Postoperative visual acuity was 4/10 in 1 case. hand motions in 3 cases and the loss of light perception in 1 case. Silicon oil was removed in 3 eyes. but not in the eye with total retinal redetachment and in the other eye with retinal redetachment involving the inferior quadrants. Conclusion : Anatomical success can be achieved by vitreoretinal surgery in most cases with lens dislocation and retinal detachment with PVR; however. visual prognosis remains limited. PVR. anterior and posterior segment complications secondary to open globe injuries and congenital glaucoma were thought to be risk factors for the limited visual prognosis.

Amaç: Vitreus içine lens dislokasyonu ile birlikte proliferatif vitreoretinopati (PVR)'li retina dekolmanı bulunan olgularda vitreoretinal cerrahinin görsel ve anatomik sonuçlarını irdelemek. Yöntem: Dördü travmaya, l'i ise konjenital glokoma ikincil lens dislokasyonu ile birlikte PVR'li retina dekolmanı olan 5 olgu retrospektif olarak incelendi. Bu olgulara pars plana vitrektomi (PPV), sıvı perflorokarbon (SPFK) yardımıyla disloke lensin okütom ile kesilerek vitreus içinde aspirasyonu ve retinanın yatıştırılması, epiretinal membran soyulması, gerekli olgularda retinektomi ve subretinal membran soyulması, 360 derece sklera çökertmesi, lazer fotokoagülasyonu ve silikon yağı injeksiyonu uygulandı. Çalışmamızda araştırdığımız ana parametreler, anatomik ve görsel sonuçlar idi. Bulgular: Yaş ortalaması 16.8 (6-32) olan olgular ortalama 7 ay (4-12 ay) takip edildi. Takip döneminde 3 olguda retina tüm kadranlarda yatışık kaldı. Bir olguda postoperatif 4. ayda retina dekolmanı alt yarıda nüks etti ve 12 aylık takip boyunca bu dekolman, maküla yatışık olmak üzere stabil kaldı. Preoperatif dönemde prefitizik durumdaki olguda ise total retina dekolmanı ve PVR gelişti. Tüm olgularda preoperatif görme keskinliği ışık persepsiyonu düzeyindeydi. Postoperatif dönemde görme keskinliği 1 olguda 4/10, 3 olguda el hareketi, 1 olguda ise ışık hissinin kaybı şeklindeydi. Nüks total retina dekolmanı gelişen olgu ve alt yarıda nüks retina dekolmanı gelişen olgu dışındaki 3 olguda silikon yağı çıkartıldı. Sonuç: Vitreus içine lens dislokasyonu ile birlikte PVR'li retina dekolmanı bulunan olgularda vitreoretinal cerrahi ile çoğu olguda anatomik başarı elde edilmesine karşın, fonksiyonel sonuçların aynı ölçüde yüz güldürücü olmadığı gözlendi. PVR, açık göz yaralanmasına ikincil ön ve arka segment komplikasyonları ve konjenital glokom varlığı görsel prognozu olumsuz etkileyen etmenler olarak düşünüldü.
