The effect of capsular tension ring on surgical complications in eyes with pseudoexfoliation syndrome during phacoemulsification surgery

Abstract

Purpose: To examine how the use of capsular tension ring (CTR) application effects the surgical complications during phacoemulsification surgery of eyes with pseudoexfoliation syndrome. Methods: Fifty eight eyes of 58 patients who had pseudoexfoliation syndrome and had undergone phacoemulsification surgery were retrospectively analyzed. The capsular tension ring was implanted after the hydrodissection stage of the surgery in 15 eyes and was not implanted in 43 eyes. Results: Among the eyes without CTR, 7 (16,2 %) developed zonular dehiscence and 11 (25,6%) developed rupture of the posterior capsule (7 of them with vitreous loss). In contrast,no eye had zonular dehiscence and only one eye had posterior capsule rupture. (6.6 %) in CTR implantation group . Zonular dehiscence occurred significantly less in eyes with CTR implan-tation (p0.05). Conclusion: The use of capsular tension ring can effectively reduce the zonular complications of the phacoemulsification surgery in eyes with pseudoexfoliation syndrome. However, the ring seems to have a limited effect on prevention of capsular complications.

Amaç: Psödoeksfoliasyon sendromlu katarakt olgularında fakoemulsifikasyon cerrahisi sırasında kapsül germe halkası (KGH) kullanımının cerrahi komplikasyonlar üzerine etkisini incelemek amaçlanmıştır. Yöntem: Psödoeksfoliasyon sendromu tanısıyla izlenen VQ katarakt cerrahisi geçiren 58 hastanın 58 gözü retrospektif olarak incelendi. Olguların 15'inde hidrodiseksiyon fazından sonra kapsül germe halkası kapsüler kese içine yerleştirilirken, 43'ünde ise yerleştirilmedi. Bulgular: KGH yerleştirilmeyen olguların 7'sinde (%16,2) zonül zaafı ve l Tinde (%25,6) arka kapsül defekti (7'sinde vitreus kaybı ile birlikte) meydana geldi. Halka yerleştirilen olgularda zonül zaafı gözlenmezken, yalnızca l olguda (%6,6) arka kapsül bütünlüğü ve vitre kaybı gözlendi. Halka yerleştirilen gözlerde zonül zaafının anlamlı olarak daha az geliştiği (p0.05). Tartışma: Psödoeksfoliasyonlu olgularda kapsül germe halkası kullanılması zonüler komplikasyonları azaltmakla beraber, arka kapsülle ilgili komplikasyonları önleyici etkisi sınırlı görünmektedir.
