The efficiency of acetazolamide in the treatment of cystoid macular edema due to uveitis

Abstract
Purpose: To evaluate the efficiency of acetazolamide in the treatment of the cystoid macular edema (CME) due to uveitis with assesment of visual acuity, fluorescein angiography (FFA) and optical coherence tomography (OCT) findings. Material and methods: Thirty four eyes of 26 patients diagnosed with Behcet's Disease or non-specific uveitis, with newly developed cystoid macular edema were included in the study. All patients underwent full ophthalmic examination, including FFA and OCT, and were treatedwith acetazolamide 250 mg 3x1 for two months (750 mg/day). The ophthalmic examination, FA and OCT were repeated every month. The corrected visual acuity and the macular thickness measured by OCT before and after treatment was compared with " t test". The CME levels monitored at FFA before and after treatment were compared with "Wilcoxon Matched Pairs Signed Ranks test". Results: There was a statistically significant increase in the corrected visual acuity (p=,000), reduction in the macular thickness measured by OGT (p=,000), reduction in the CME level monitored at FFA (p=,000) after the treatment. Discussion: OCT provides non-invasive and quantitative imaging compared to invasive and qualitative method as FFA in diagnosing and following treatment of CME. Although larger and randomized studies are required in order to prove the efficiency of acetazolamide in treat-ment of cystoid macular edema due to uveitis, acetazolamide could be an alternative to the other methods in treating newly developed CME.

Amaç: Üveite bağlı kistoid makula ödeminin (KMÖ) tedavisinde asetazolamidin etkinliğinin görme keskinliği, flöresein anjiografi (FFA) ve optik koherens tomografi (OKT) bulguları ile araştırılması. Gereç ve Yöntem: Behçet hastalığı ya da özgül olmayan üveit tanısı ile takip edilmekte olan 26 olgunun 34 gözü yeni gelişmiş kistoid makula Ödemi ön tanısı ile çalışmaya dahil edildi. Olgulara tam bir göz muayenesi yapıldı. FFA ve OKT uygulandı. Tümünde KMÖ tespit edilen hastalara 2 ay süre ile asetazolamid 250 mg. tablet 3x1 olarak verildi. Birer ay ara ile oftalmolojik muayene, FFA ve OKT tekrarlandı. Tedavi öncesinde ve sonrasında düzeltilmiş görme keskinlikleri ve OKT ile belirlenen makula kalınlıkları "t testi", FFA'da tespit edilen KMÖ dereceleri "Wilcoxon Matched-Pairs Signed Ranks" testi ile karşılaştırıldı. Sonuçlar: Tedavi sonrasında düzeltilmiş görme keskinliğinde anlamlı artış (p=,000), OKT ile belirlenen makula kalınlığında anlamlı azalma (p=,000), FA'da izlenen KMÖ derecesinde anlamlı azalma (p=,000) saptandı. Tartışma: OKT, KMÖ tanısında ve takibinde invaziv ve kalitatif bir yöntem olan FA'ya göre non-invaziv ve kantitatif görüntüleme sağlar. Asetazolamidin Üveite bağlı kistoid makula ödemi tedavisinde etkinliğinin kanıtlanması için daha geniş ve randomize çalışmalara ihtiyaç olmakla birlikte özellikle yeni gelişmiş KMÖ'de diğer tedavi yöntemlerine alternatif olabileceğini düşünmekteyiz.
