Corresponse of posterior capsule opacification in acrylic and PMMA intraocular lenses in long period

Abstract
Purpose: The most common surgically related cause of reduced vision after extracapsular cataract extraction (ECCE) is posterior capsule opacification (PCO). It occurs in up to 50% of eyes following cataract extraction. This study was aimed at comparing the percentage of PCO and laser capsulotomy rates with polymethylmethacrylate (PMMA) and acrylic intraocular lens (IOL) implants 4 years postoperatively.Methods: In this retrospective study, 1380 eyes that had undergone standardized phacoe-mulsification with 4.5 to 5.0 mm capsulorhexes and capsular bag 5.5 mm 3-piece foldable acrylic IOL implantation (Group A, n: 747 eyes) or 5.5 mm rigid PMMA IOL (Group B, n: 633 eyes) were evaluated for the PCO development. Results: Group A (4.41%) differed from Group B (46.9%) in the lower incidence of PCO development and the lower Nd YAG Laser capsulotomy rates. At 4 years postoperatively, the percentage of PCO development 46.9 percent of patients with PMMA lOL's, 96.9% percent of those required a Nd: YAG Laser capsulotomy, whereas 4.41 percent of patients with acrylic lenses developed PCO and neither of them needed a capsulotomy. Conclusions: PCO after IOL implantation has a multifactored pathogenesis. Capsular bag implantation of 5.5 mm acrylic IOL are likely to reduce the PCO incidence significantly. lOL's made of PMMA were associated with a significantly higher degree of PCO and more laser capsulotomies than poly aery lie intraocular lenses. Providing better materials and creating functional designs improve patient outcomes following cataract surgery. Biocompatibility and reduced PCO are among the features of the newly generated lOL's.

Amaç: Ekstrakapsüler katarakt cerrahisi (EKKE) sonrası cerrahi ile ilişkili düşük görme nedenleri arasında en sık görülen sebep arka kapsül kesifleşmesidir. Katarakt cerrahisi sonrası %50 oranına dek görülebilmektedir. Bu çalışmada PMMA ve hidrofobik akrilik lens implantlı hastalarda arka kapsül kesifleşmesi ve laser kapsülotomi (LK) yapılma oranlan postoperatif 4 yıllık süreçte araştırılmıştır. Yöntem: Bu retrospektif çalışmada, 4.5 - 5.0 mm kapsüloreksisli, standart fakoemülsifikasyon yapılmış ve 5.5 mm 3-parçalı katlanabilir hidrofobik akrilik GİL (göz içi lensi) (Grup A, n: 747 göz) yada 5.5 mm sert PMMA GİL (Grup B, n:633 göz) implante edilmiş hastalar, arka kapsül kesifleşmesi açısından değerlendirilmiştir. Bulgular: Arka kapsül kesifleşmesi gelişme insidansı ve Nd:YAG laser kapsülotomi oranları Grup A'da (%4.41), Grup B'den (%46.9) çok daha düşüktür. Postoperatif 4. yılda arka kapsül kesifleşmesi gelişme oranı PMMA GİL grubunda %46.9 olup, bunların %96.9 una Nd:YAG LK yapılmışken, akrilik lens implante edilen hastalarda %4.41 oranında arka kapsül kesifleş-miştir ve hiçbiri laser kapsülotomi gerektirmemiştir. Tartışma: GİL implantasyonu sonrası arka kapsül kesifleşmesi patogenezinde birçok faktör sorumludur. Çalışmada 5.5mm akrilik GİL'lerin kapsül içine implantasyonunun arka kapsül kesifleşmesi insidansını önemli derecede azalttığı görülmektedir. PMMA'dan yapılan GİL'ler, akrilik lenslere oranlara belirgin derecede yüksek arka kapsül kesifleşmesine ve laser kapsüloto-mi gereksinimine neden olmaktadır. Daha iyi GİL materyallerinin geliştirilmesi ve optik kenar köşeliliği gibidaha fonksiyonel lens tasarımlarının katarakt cerrahisi sonrası arka kapsül kesifliği gelişimini önlemede olumlu katkılarda bulunacağı açıkça gözükmektedir.Yeni jenerasyon GİL'lerde de ilk hedef biyouyumluluk ve azalmış arka kapsül kesifliği oranları olacaktır.
