Abstract
Effect of the 5.5 mm 3 Piece Acrylic IOL at the Posteor Capsule Opacification: Our Results of Randomized Prospective Study During Three Years Purpose: Posterior capsule opacification (PCO) is the most common complication of cataract surgery occurring in up to 50% of patients by 2 to 3 years after the operation. This study was aimed at determining if the 5.5 mm 3 piece acrylic IOL is effective on preventing PCO development Methods: In this prospective study, 812 eyes of 699 patients underwent standardized pha-coemulsification with 4.5 to 5.0 mm capsulorhexes and capsular bag 5.5 mm 3-piece foldableacrylic IOL implantation. The patients were randomly assigned to be polished the anterior capsules or not. Retroillumination photographs were taken at 6 months and then yearly. Results: Throughout the follow-up, patients with 5.5 mm Acrylic IOL implantations showed a minor number of PCO and neither of them required Nd YAG laser capsulotomy. Anterior capsule polishing does not seem to have any effect on PCO development. Conclusions: Multiple pathogenesis proposed for PCO after IOL implantation. Capsular bag implantation of 5.5 mm acrylic IOL are likely to reduce the PCO incidence significantly. Advanced materials and designs improve patient outcomes following cataract surgery. Biocom-patibility and reduced PCO are among the features of the latest generation IOLs.The significan-ce of the IOL optic diameter in association with the capsulorhexis size, lens design and material should also be documented by further studies.
Özet
Amaç: Arka kapsül kesifliği (AKK), ekstrakapsüler katarakt cerrahisini takiben 2-3 yıl içerisinde hastaların %50'sinde görülebilen bir komplikasyondur. Bu çalışmada 5.5 mm 3-parçalı akrilik lens implante edilen hastalarda AKK gelişimini araştırdık. Yöntem: Bu prospektif çalışma,standart fakoemülsifikasyon ve 4.5-5.0 mm kapsüloreksis ve kapsül içi 5.5 mm 3-parçalı akrilik GİL implante edilen 812 hastanın 699 gözü kapsamaktadır. Hastalar randomize olarak ön kapsül polishing yapılanlar ve yapılmayanlar olarak iki gruba ayrıldılar. Retroillüminasyon fotoğrafları önce 6. ayda takiben yıllık olarak alındı. Elde edilen veriler khi kare testi ile değerlendirildi. Sonuçlar: İzlem süreci boyunca 5.5 mm akrilik GİL implante edilen hastalarda, hiçbirinde ND YAĞ Laser kapsülotomi gerektirmeyen, çok düşük sayıda AKK görüldü. Ön kapsül polis-hing yönteminin AKK gelişimi açısından herhangi bir etkisi saptanmadı. Tartışma: GİL implantasyonunu takiben AKK gelişiminde çok sayıda patogenez önesürül-müştür. 5.5 mm akrilik GİL'lerin kapsül içi implantasyonları AKK insidansında belirgin düşüşe neden olmaktadır. Katarakt cerrahisi sonrası geliştirilmiş materyaller ve tasarımlar sonucu elde edilmiş lenslerin hasta beklentilerine olumlu katkıları olacaktır. Biyouyumluluk ve azalmış AKK son nesil lenslerin özellikleri arasında başta gelmektedir. GİL optik çapı ve kapsüloreksis boyutu, lens materyali ve tasarım özellikleri gibi parametrelerin AKK üzerine etkinlikleri karşılaştırmalı çalışmalar ile irdelenmelidir.
