Duane syndrome associated with ipsilateral peripheral facial paralysis and crocodile tears: A case report

Abstract

Purpose: To present a case of Duane's Syndrome associated with İpsilateral peripheral fa-cial paralysis and crocodile tears. Methods: A four-year-old female patient with a history of head tilt and eye movement disorder starting from birth was brought to our department. Detailed ophthalmological and systemic examinations were performed. Results: In her ophthalmological examination, she had left esotropia with abduction deficit, globe retraction and narrowing of palpebral aperture during adduction (Duane's Syndrome Type I). Her face is tilted to left. Biomicroscopical and fundus examinations were within normal limits. Peripheral type facial paralysis was observed in the left part of her face. She had tearing in left eye during chewing (crocodile tears). Brain tomography was reported to be normal.Conclusion: A variety of systemic and ocular malformations may accompany Duane's Syndrome. Hence, every patient with this disorder should be evaluated carefully for the possible presence of associated anomalies.

Özet

Amaç: İpsilateral periferik fasial paralizi ve timsah gözyaşı sendromunun eşlik ettiği bir Duane sendromu olgusunu sunmak. Yöntem: Kliniğimize doğuştan beri mevcut olduğu söylenen baş pozisyonu ve göz hareketi kısıtlılığı ile getirilen 4 yaşındaki bir kız çocuğu ele alındı. Detaylı oftalmolojik ve sistemik muayeneleri yapıldı. Bulgular: Yapılan oftalmolojik muayenede; primer pozisyonda sol ezotropya, sol gözde abdüksiyon kısıtlılığı, addüksiyonda glob retraksiyonu ve kapak aralığında daralma (Duane Tip-I) tespit edildi. Baş sola deviyeydi. Ön segment bulguları ve fundus muayenesi normal sınırlarda tespit edildi. Yüzün sol kısmında periferik fasial paralizi bulguları tespit edildi. Yemekle beraber sol gözde yaşarma gözlendi (timsah gözyaşı). Beyin tomografisi normal sınırlarda olarak rapor edildi. Tartışma: Duane sendromuna sıklıkla çeşitli oküler ve sistemik malformasyonlar eşlik etmektedir. Her hastanın ayrıntılı bir şekilde sistemik açıdan değerlendirilmesi gerekmektedir.
