Effect of latanoprost pretreatment on the intraocular pressure elevation following YAG-laser posterior capsulotomy

Abstract
Aim: To investigate the effect of prophylactic latanoprost treatment on the intraocular pressure elevation following YAG laser posterior capsulotomy.Material-Method: Patients scheduled for YAG laser capsulotomy for posterior capsule opacification secondary to cataract extraction with or without intraocular lens implantation were randomly classified into latanoprost treatment and control groups. Twenty-two eyes in the treatment group, received one drop of 0.005% latanoprost after measurement of the basal intraocular pressure (IOP), after one hour IOP measurement was repeated and 1% tropicamide was used for pupil dilation. After approximately 45 minutes, YAG laser posterior capsulotomy was performed and post-laser first and second hour IOP were measured. For the 23 eyes of the control group, the same procedure was applied except for application of latanoprost, Results: Mean of ages were 59.1 ±21.6 in the latanoprost treatment group and 59.3±21.8 in the control group. Indications for YAG laser capsulotomy mostly included posterior capsule opacification secondary to extracapsular cataract extraction and intraocular lens implantation. The time between cataract surgery and the laser capsulotomy, laser spot number, laser spot energy, basal and post-laser first and second hour IOP levels did not show statistical difference between the groups (p>0.05). In the control group, the difference between post-laser first hour IOP and basal IOP was statistically significant (p=0.044) while the difference between post-laser first hour IOP and post-latanoprost IOP was significant in the latanoprost treatment group (p=0085). Conclusion: In contrary to the control group, there was no significant difference between post-laser first hour IOP and basal IOP in the latanoprost treatment group. This result was considered to be important especially for patients for whom short term high IOP levels might be harmful.

Amaç: YAG-laser arka kapsülotominin yolaçtığı göziçi basınç (GİB) artışına, profilaktik olarak uygulanan latanoprostun etkisinin araştırılması amaçlanmıştır. Materyal-Metod: Katarakt cerrahisi sonrası gelişen arka kapsül kesafeti nedeniyle YAG-laser kapsülotomi planlanan hastalar, tedavi grubu (22 hasta) ve kontrol grubu (21 hasta) olarak rastgele ayrıldı. Tedavi grubundaki 22 göze, ilk GİB alındıktan sonra latanoprost l damla uygu-landı, l saat sonra GİB ölçümünü takiben, pupilla dilatasyonu için tropikamid l damla uygulan-dı, yaklaşık 45 dakika sonra laser tedavisi yapıldı, laser sonrası 1. ve 2. saatlerde GİB ölçümü tekrarlandı. Kontrol grubundaki 23 göze, latanoprost kullanımı dışında, aynı prosedür uygulandı. GİB'ları, katarakt cerrahisinden itibaren geçen süre, tanı, laser atım sayısı ve enerjisi açısından gruplar karşılaştırıldı. Bulgular: Yaş ortalaması, latanoprost tedavisi grubunda 59.7±21.6, kontrol grubunda 59.3±21.8 idi. Kapsülotomi endikasyonlarının çoğunluğunu ekstrakapsüler katarakt ekstraksi-yonu ve arka kamara lens implantasyonu sonrası gelişen arka kapsül kesafeti oluşturuyordu. Latanoprost tedavisi grubu ile kontrol grubu karşılaştırıldığında, katarakt cerrahisinden sonra geçen süre, laser atım sayısı, enerji, ilk GİB, laser sonrası 1. ve 2. saat GİB'ları arasında anlamlı farklılık izlenmedi (p>0.05). Kontrol grubunda 1. saat GİB ile ilk GİB arasındaki fark istatistiksel olarak anlamlı bulunurken (p=0.044), latanoprost tedavisi grubunda latanoprost sonrası GİB ile post-op 1. saat GİB arasındaki fark anlamlıydı (p=0.0085). Sonuç: Latanoprost uygulaması sonrası yapılan laser kapsülotominin 1. saatinde elde edilen GİB değerleri, kontrol grubunun aksine, ilk GİB değeri ile anlamlı farklılık oluşturmamıştır. Bu bulgu, özellikle kısa süreli GİB artışlarından olumsuz etkilenebilecek olgular açısından kayda değer olarak düşünülmüştür.
