Controlling of intraocular pressure in patients with coexisting glaucoma and cataract

Abstract

Purpose : To examine the effects of phacoemulsification and phacotrabeculectomy techniques on intraocular pressure (IOP) in patients with coexisting cataract and POAG. Methods : 18 eyes of 15 patients with POAG and cataract were included in this study, 11 eyes were undergone phacoemulsification+posterior chamber intraocular lens (PCIOL) implantation and 7 eyes were undergone phacotrabeculectomy+ posterior chamber intraocular lens (PCIOL) implantation. Visual acuity, IOP, visual fields were measured and number of glaucoma medications were noted preoperatively. Patients were examined on the postoperative 1.day and after 1 week, 1 month, than every 3 months. Postoperative follow-up criteries were IOP and visual acuity. Results : Mean follow-up period was 27.18 months and mean age was found 71.4 years in phacoemulsification+PCIOL implantation group and 75.8 years in phacotrabeculectomy+PCIOL implantation group. Mean preoperative and postoperative (6.month) best corrected visual acuity was found 0.3 and 0.74 in phacoemulsification +PCIOL implantation group while 0.37 and 0.68 in phacotrabeculectomy+PCIOL implantation group. Mean preoperative and postoperative IOP in 1, 3, 6, 12, months were 17.6±2.2 mmHg 14.3±2.6mmHg, 14.7±2.3mmHg, 15.1±2.2mmHg, 16.5±2.1mmHg in phacoemulsification+PCIOL implantation group. In phacotrabeculectomy+PCIOL implantation group IOP in 1.3.6.12, months were 24.3±4.9mmHg, 15.9±4.9mmHg, 15.0±4.8mmHg, 12.9±4.7mmHg, 14.1±2.6mmHg. Each post-operative IOP values were lower than pre-operative values in both two groups. The difference between initial and final IOP in the first group was statistically significant on 1, 3, and 6, months (Wilcoxon test p= 0.021, p =0.04, p= 0.027) In the second group difference between initial and final IOP was statistically significant on 1, 6, and 12, months (Wilcoxon test p= 0.028, p= 0.028, p= 0.018). Conclusion : Phacoemulsification and posterior chamber IOL implantation without trabeculectomy also decreases intraocular pressure in patients with coexisting glaucoma and cataract.

Amaç: Primer açık açılı glokomu (PAAG) ve kataraktı olan olgularda fakoemulsifikasyon ile katarakt ekstraksiyonunun veya fakotrabekülektominin göz içi basıncına (GİB) olan etkisinin incelenmesi. Metod: PAAG ve kataraktı olan 15 hastanın 18 gözü çalışmaya alındı. 11 göze fakoemulsifikasyon + göz içi lens (GİL) implantasyonu, 7 göze fakotrabekülektomi+ göz içi lens (GİL) implantasyonu uygulandı. Preoperatif görme keskinliği, GİB, görme alanı muayenesi ve anti glokomatöz ilaç sayısı değerlendirme kapsamına alındı. Postoperatif 1. gün, 1. hafta, 1.ay ve daha sonra 3'er ay ara ile hastalar takip edildi. Postoperatif dönemde değerlendirme parametreleri, görme keskinliği ve GİB idi. Bulgular: Çalışma boyunca ortalama takip süresi 27.18 ay olup fakoemulsifikasyon + GİL implantasyonu yapılan grupta yaş ortalaması 71.4, fakotrabekülektomi + GİL implantasyonu yapılan grubta 75.8 yıl olarak bulundu. Sadece katarakt ekstraksiyonu yapılan grupta preoperatif ortalama görme keskinliği 0.30, postoperatif (6.ay) 0.74 iken, fakotrabekülektomi yapılan ikinci grupta preoperatif ortalama keskinliği 0.37, postoperatif (6.ay) 0.68 idi. Birinci grupta ortalama GİB preoperatif 17.6±2.2 mmHg olup post-operatif 1., 3., 6. ve 12.aylarda sırasıyla 14.3±2.6mmHg, 14.7±2.3 mmHg, 15.1±2.2 mmHg, 16.5±2.1mmHg olarak bulundu. İkinci grupta preoperatif ortalama GİB 24.3±4.9mmHg, postoperatif 1., 3., 6. ve 12. aylarda 15.9±4.9mmHg, 15.0±4.8mmHg, 12.9±4.7mmHg, 14.1±2.6mmHg bulundu. Cerrahi sonrası her iki grupta da tüm ölçümler başlangıç değerleri ile kıyaslandığında daha düşük saptandı. Postoperatif GİB değerleri preoperatif GİB değerleri ile kıyaslandığında aradaki fark birinci grupta 12. ay hariç 1.3. ve 6. aylarda (Wilcoxon testi p= 0.021, p =0.04, p= 0.027), ikinci grupta 3. ay hariç 1.6. ve 12. aylarda istatiksel olarak anlamlıydı (Wilcoxon testi p= 0.028, p= 0.028, p= 0.018). Sonuç: Glokomu olan katarakt olgularında trabekülektomi yapılmaksızın sadece fakoemulsifikasyon yöntemiyle katarakt ekstraksiyonu sonrasında da GİB'de düşme saptanmıştır.
