Thyroid dysfunction and glaucoma

Abstract
Purpose: Glaucoma incidence in patients with thyroid dysfunction was investigated and the effects of the treatment for thyroid dysfunction on glaucomatous findings were evaluated. Method: One hundred three patients (40 euthyroid goitre, 30 hyperthyroid, 22 hypothyroid, 10 subclinic hypothyroid, 1 subclinic hyperthyroid) who were diagnosed as thyroid dysfunction in the Endocrinology Clinic of our hospital, were screened for glaucoma by means of visual acuity tests, anterior and posterior segment examinations, intraocular pressure (IOP) measurements, gonioscopic examinations and automated visual field (VF) tests in our Glaucoma Department. IOP measurements and VF tests were repeated in patients with signs of glaucoma after two months of specific therapy for their thyroid dysfunction. Results: Normotensive glaucomatous findings were present in 5 of 40 patients with euthyroid goitre, in 8 of 30 patients with hyperthyroidism, in 5 of 22 patients with hypothyroidism, and two of 10 patients with subclinical hypothyroidism, totally in 20 cases (19.4%). After two-month therapy for thyroid dysfunction, IOP levels remained unchanged, however visual field mean deviation and corrected pattern standard deviation parameters were significantly improved in all Discussion: It is emphasised that, glaucomatous damage of the optic nerve and probably retinal ganglion cells might occur in cases with thyroid dysfunction, by various mechanisms, independent of IOP and this damage might be controlled or reversed by the treatment of the thyroid dysfunction.

Amaç: Tiroid disfonksiyonu bulunan hastalarda glokom insidansının ve tiroid disfonksiyonuna yönelik tedavinin glokomatöz bulgular üzerindeki etkisinin araştırılması. Yöntem: Ankara Numune Eğitim ve Araştırma Hastanesi 1. Göz Kliniği Glokom Biriminde, hastanemizin Endokrinoloji Kliniğinde ilk kez tiroid disfonksiyonu tanısı almış 103 hasta (40 ötiroid, 30 hipertiroid, 22 hipotiroid, 10 subklinik hipotiroid, l subklinik hipertroid) glokom varlığı açısından değerlendirildi. Her hastaya görme keskinliği, ön-arka segment muayeneleri, gonioskopi ve bilgisayarlı görme alanı (BSGA) testleri uygulandı ve göziçi basınç (GİB) ölçümleri yapıldı. Glokomatöz bulgular gözlenen olguların GİB ölçümleri ve BSGA testleri, yalnızca tiroid disfonksiyonlarına yönelik tedavi aldıkları 2 ayın sonunda tekrarlandı. Sonuçlar: Değerlendirilmeye alınan 40 ötiroidili hastanın 5'inde, 30 hipertiroidili hastanın 8'inde, 22 hipotiriodili hastanın 5'inde ve 10 subklinik hipotiroidili hastanın 2'sinde olmak üzere toplam 20 hastada (%19.4), normotensif sınırlarda glokomatöz bulgular saptandı. Tüm tiroid disfonksiyonu alt gruplarında, 2 aylık tedavi sonrasında tekrarlanan GİB ölçümlerinde istatistiksel olarak anlamlı bir değişim saptanmazken, BSGA; ortalama sapma ve düzeltilmiş patern standart sapma değerlerinde istatistiksel olarak anlamlı düzeyde olumlu değişim gözlendi. Tartışma: Tiroid disfonksiyonlu olgularda GİB'dan bağımsız mekanizmalarla optik disk ve retina gangliyon hücrelerinde glokomatoz özellikte hasar ya da fonksiyon bozukluğu meydana gelebildiği ve bu durumun tiroid disfonksiyonunun tedavisi ile durdurulabildiği veya geri döndürebildiğine dikkat çekildi.
