The risk of glaucoma development in eyes with pseudoexfoliation syndrome

Abstract
Purpose : This study aims at investigation of the risk of increase in intraocular pressure (IOP) and development of pseudoexfolative glaucoma (XFG) in cases with pseudoexfolation syndrome (XFS). Methods : 386 eyes of the 247 cases at Trakya University. Faculty of Medicine. Ophthalmology Department. Glaucoma Unit have been assessed retrospectively for this purpose, and 94 eyes of 75 cases which have been diagnosed with XFS at first application with a minimum observation period of 2 years have been selected. Eyes with an IOP of over 21 mmHg have been evaluated as XFS and high IOP and eyes from among these with high IOP, having glaucomatous visuel field and optic disc changes, have been evaluated as XFG during the observation. Results : The mean observation period for the cases included into the study was 66±30 months. During the observation, it has been determined that IOP increased in 25 of the 94 eyes with XFS (27%) and that XFG developed in 5 cases (5%). The Cox regression analysis revealed the risk of increase in IOP in the eye with XFS at the end of the 105 months as 60% and the risk of XFS development was found as 10% after 72 months. Conclusion : XFS gradually turns into XFG at significant rates. Cases diagnosed with XFS must be under regular observation in order to prevent the glaucomatous damages caused by XFG.
Özet
Amaç: Bu çalışmanın amacı psödoeksfoliasyon sendromlu (PES) olgularda göz içi basıncı (GİB) yükselme ve psödoeksfoliatif glokom (PEG) gelişme riskini araştırmaktır. Yöntem: Bu amaçla Trakya Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Glokom biriminde izlenen 247 psödoeksfoliasyonlu olgunun 386 gözü retrospektif olarak değerlendirildi ve bunların arasından ilk başvurularında PES tanısı almış, en az 2 yıl izlemi olan 75 olgunun 94 gözü değerlendirmeye alındı. İzlemi sırasında GİB'si 21 mmHg'mn üzerine çıkan gözler PES ve yüksek GİB olarak, bunlar arasından glokomatöz görme alanı ve optik disk değişikliklerinin de saptandığı gözler PEG olarak değerlendirildi. Sonuçlar: Çalışmaya alınan olguların ortalama izlem süresi 66±30 ay idi. İzlem sürecinde PES'li 94 gözden 25'inde (%27) GİB yükseldiği ve 5'inde de (%5) PEG geliştiği saptandı. Cox regresyon analizi ile değerlendirme yapıldığında; PES'li bir gözde 105 ay sonunda göz içi basıncı yükselme riski %60 ve 72 ay sonunda PEG gelişme riski %10 olarak bulundu. Tartışma: PES zaman içerisinde önemli oranlarda PEG'ye dönüşmektedir. PEG'nin neden olacağı glokomatöz hasarı önleyebilmek için PES'li olguların düzenli aralıklarla izlenmesi gerekmektedir.
