The effect of corneal incision size on surgically induced astigmatism in phacoemulsification surgery

Abstract
Purpose: To evaluate the effects of different corneal incision diameters and sutures on surgical astigmatism.Materials and Method: 60 eyes of 60 patients were included inthis study and divided into 3 groups. Group 1 consisted of 20 cases with 3 mm clear corneal incision, Group 2 consisted of 20 cases with 5 mm clear corneal incision and Group 3 consisted of 20 cases with 5 mm clear corneal incision and a corneal suture. All patients had pre and post operative (Day 1 and 30) keratometry. The surgically induced astigmatism and changes of keratorhetric values were calculated by the vector analysis method.Results: There was no statistically significant difference between 3 groups in surgically induced astigmatism. Conclusion: In cases where corneal incision has to be widened or o foldable intraocular lens cannot be implanted, 5 mm clear corneal incision can be done quiet easily and good postoperative results can be achieved

Amaç: Fakoemülsifikasyonlu katarakt cerrahisinde sütürsüz 3 mm ve 5 mm kornea kesileri ile 5 mm tek sütürlü kornea kesilerinin post operatif dönemde cerrahi astigmatizma üzerine etkisinin değerlendirilmesi Gereç ve Yöntem: Kliniğimize katarakt cerrahisi için başvuran ve altta yatan başka herhangi bir oftalmolojik problemi olmadığı rutin muayeneler sonucu kesinleştirilen 60 hastanın 60 gözü çalışmaya dahil edildi. Bunların 20'sine 3 mm sütürsüz insizyon, 20'sine 5 mm sütürsüz insizyon ve 20'sine 5 mm tek sütürlü şeffaf superior korneal insizyon uygulanarak standart fakoemülsifikasyon cerrahisi uygulandı. Keratometre ölçümleri ameliyat öncesi, ameliyat sonrası 1. gün ve 1. ayda tekrarlandı. Cerrahi sonucu gelişen astigmatizma miktarı ve aks değişikliği; vektör analizi metodu ile değerlendirildi. Bulgular: 3 mm ve 5 mm sütürsüz insizyon yapılan vakalar ile 5 mm tek sütürlü insizyon yapılan vakalar arasında, postoperatif dönem astigmatizma oranları açısından anlamlı bir fark gözlemlenmedi. Sonuç: Korneal tünel insizyonun herhangi bir nedenle genişletilmesi gerektiği ve katlanabilir lens implantasyonunun mümkün olmadığı vakalarda 5 mm korneal insizyonun rahatlıkla tercih edilebileceğini ve iyi sonuçlar alınabileceğini düşünüyoruz.
