Reasons and outcome of incomplete flap in LASIK: In view of a case

Abstract

Twenty-two year old female patient presented to our clinic asking for the correction of her refractive error. Her examination revealed small palpebral aperture, a spherical error of -4.50 D in the right and -4.25 D in the left eye with cylindrical error of -0.50 D at 140-dcgree meridian. The remaining ophthalmic features were within normal limits. Pachymetry showed the thickness of cornea as 622 in the right and 616 urn in the left eye. An uneventful LASIK procedure was performed in the right eye. In the left eye, at the end of the suction and flap creation, we observed an irregular, incomplete flap in the central cornea. The procedure was terminated and LASIK postponed. During follow-up, a scar in the paracentral cornea and an increase in the cylindrical value developed. Five months following the first event we performed LASIK with a 180 um flap thickness. The uncorrected visual acuity increased to 1.0 at the postoperative one month. Irregular and incomplete flap is a frightening complication of LASIK procedure. However, after the completion of wound healing an acceptable result can be obtained after the second procedure.

22 yaşındaki bayan hasta kliniğimize refraksiyon kusurunun düzeltilmesi isteğiyle başvurdu. Muayenesinde dar kapak aralığı, sağda -4,50 D, solda -4,25 D değerlerinde sferik ve 140 derecelik aksta -0,50 D silindirik kırılma kusuru saptandı. Diğer oküler özellikleri normal sınırlardaydı. Pakimetrisinde kornea kalınlığı sağda 622 solda 616 um düzeyinde ölçüldü. Sağ gözüne komplikasyonsuz LASİK girişimi uygulandı. Sol gözde vakum ve flep oluşturma sonrasında kornea santralinde sınırları düzensiz tam olmayan flep oluştuğu görüldü. Hastanın işlemi durduruldu ve LASİK girişimi ertelendi. Takiplerinde kornea parasantralinde skarı ve astigmat değerinde bir artış olduğu görüldü. İlk olaydan 5 ay sonra 180 um kalınlığında flep ile LASİK tekrarlandı. Cerrahi sonrası birinci ayda sonunda düzeltilmemiş en iyi görmesi 1,0 (tam) düzeye çıktı. Düzensiz ve tam olmayan flep LASİK cerrahisinin ürkütücü bir komplikasyonudur. Ancak, yara yeri iyileşmesinden sonra ikinci girişim ile kabul edilebilir bir sonuç elde edilebilir.
