Results of porous polyethylene sphere as an anopthalmic orbital implant

Abstract

Purpose: To investigate the results of porous polyethylene sphere as an anopthalmic orbital implant. Materials & Method: 61 cases to whom porous polyethylene sphere were implanted as an orbital anophthalmic implant were included in this study. This study was implemented in.and oculoplastic surgery section between the period of October 2001 and February 2003. Porous polyethylene sphere was implanted to 55 cases just after the evisceration, to 3 cases after the enucleation and finally to 3 cases as a secondary implant. The average age of patients was 37,6 years ( between 3 and 75 years). Following the evisceration, the sclera was cut into two parts just below the medial and lateral rectus muscles till the optic nerve and porous polyethylene sphere was implanted. After the enucleation and during the secondary implantation, porous polyethylene sphere was not covered with any material. Size of porous polyethylene spheres which were implanted were as follows; 1 case (%1): 22 mm, 18 cases (%30): 20 mm, 41 cases (%67): 18 mm and 1 case (%1): 16 mm. Result: Average follow-up period was 18.2 months (12-38 months). Specially designed eye prosthesis was fitted successfully to all cases. Motility peg was not used in any of the cases. Neither implant exposure nor implant rejection was observed. After the operation, in one case conjunctival dehiscense and in another case chemosis which lasted more than one month were observed. Although specially designed eye prothesis was fitted, upper sulcus deformity was observed in 3 patients, ptosis and lower lid retraction were observed in 2 patients separately. Conclusion: Porous polyethylene sphere as an anophthalmic orbital implant has successful results after enucleation and evisceration operations or as a secondary implant.

Amaç: Anoftalmik orbital implant olarak poröz polietilen sfer uygulanımının sonuçlarını incelemek.Gereç ve yöntem: Ekim 2001 ve Şubat 2003 tarihleri arasında. Bölümünde anoftalmik orbital implant olarak poröz polietilen sfer uygulanan 61 olgu çalışma kapsamına alındı. 55 olguya eviserasyon, 3 olguya enükleasyon sonrasında ve 3 olguya ise sekonder implant olarak poröz polietilen sfer uygulandı. Olguların yaşları 3 ile 75 yıl arasında (ortalama 37.6 yıl) idi. Eviserasyon sonrası sklera iç ve dış rektus kaslarının hemen altından optik sinire kadar iki parçaya ayrıldı ve poröz polietilen sfer implante edildi. Enükleasyon sonrası ve sekonder implantasyonlarda poröz polietilen sfer herhangi bir madde ile sarılmadı. 1 olguya (%1) 22 mm'lik, 18 olguya (%30) 20 mm'lik, 41 olguya (%67) 18 mm'lik ve 1 olguya (%1) da 16 mm'lik poröz polietilen sfer uygulandı. Bulgular: Ortalama takip süresi 18.2 ay (12-38 ay) idi. Olgulara hareket pimi uygulanmadı. Tüm olgulara özel imal edilmiş göz protezi başarı ile uygulandı. Sklera açılması ya da implant atılımı izlenmedi. Ameliyat sonrası birer olguda konjuktiva açılması ve bir ayı geçen kemozis gözlendi. Özel imal edilmiş göz protezinin uygulanmasından sonra 3 olguda üst kapakta sulkus deformitesi, 1 olguda alt kapak retraksiyonu ve 1 olguda üst göz kapağı pitozu gözlendi. Sonuç: Anoftalmik orbital implant olarak poröz polietilen sfer eviserasyon, enükleasyon ameliyatları sonrasında ya da sekonder implant olarak yüz güldürücü sonuçlara sahiptir.
