Phacoemulsification cataract extraction and posterior chamber intraocular lens implantation in patients with uveitis

Abstract

Purpose: To investigate the safety and efficacy of phacoemulsification cataract extraction with intraocular lens implantation in eyes affected by uveitis. Methods: Fortytwo eyes of 36 patients with uveitis were retrospectively studied after phacoemulsification cataract extraction and posterior chamber lens implantation at the SSK Istanbul Education Hospital froml998 to 2003. Patients with less than 3 monts of follow up were excluded.Average follow up was 26 months (range, 4 to 60 months). Postoperative complications,degree of postoperative inflamation and final vision acuity were the main outcome measures. Results: Diagnosis of uveitis included Behçet's Disease (12 patients), İdiopathic Uveitis (6 patients), Fuch's Heterochromic İridosclitis (5 patients), Ankylosing Spondylitis (4 patients), Sarcoidosis (4 patients), Tuberculosis (2 patients), Diabetes Mellitus (2 patients), Rheumatoid Arthritis (1 patients). Final visuel acuity improved in 36 eyes. No improvement in visual acuity was seen in 6 eyes.Posterior capsular opacification was the most frequent complication in the anterior segment (17 eyes). Other postoperative complications included anterior capsular fibrin reaction (8 eyes), cystoid macular edeme (6 eyes), recurrens of uveitis (8 eyes), posterior synechiae (8eyes), antarior synechiae (4eyes), postoperative elevation of intraocular pressure (6eyes), capsular contraction (2 eyes), epiretinal membrane formation (1 eye), iris capture (1 eye). Endophthalmitis was described in one patient. Conclution: Visual results of cataract surgery depend on different uveitic entities and on posterior segment abnormalities. With careful patient selection,appropriate preoperative preparation, diligent surgery and close postoperative supervision, phacoemulsification cataract extraction with posterior chamber intraocular lens implantation is safe and effective in patientswith uveitis.

Özet

Amaç: Fakoemülsifikasyon yöntemi ile katarakt ekstraksiyonu yapılan ve intraokuler lens implantasyonu uygulanan üveitli hastalarda yöntemin güvenliğini ve etkinliğini değerlendirmek. Gereç ve Yöntem: 1998-2003 yılları arasında, Uvea-Behçet birimimizde takip edilen ve fakoemülsifikasyon katarakt ekstraksiyonu ve kapsül içi intraokuler lens implantasyonu yapılan 36 hastanın 42 gözü retrospektif olarak incelendi. Preoperatif hazırlanma protokolüne göre hastaların operasyondan önce en az 3 ay atak geçirmemiş olmalarına dikkat edildi. Postoperatif 3 aydan daha az takip edilen hastalar çalışmaya alınmadı. Ortalama takip süresi 26 ay (4-60 ay). Postoperatif komplikasyonlar, inflamasyon şiddeti ve vizüel aktiviteleri değerlendirildi. Bulgular: Etyolojik incelemelerine göre 36 hastanın 12'sinde Behçet Hastalığı, 6'sında İdiopatik Üveit, 2'sinde Tüberkülozis, 5'inde Fuch's Heterokromik İridosiklitis,l'inde Romatoid Artrit, 2'sinde Diabetes Mellitus, 4'ünde Ankilozan Spondilit, 4'ünde Sarkoidozis mevcuttu. 42 gözün 36'sında postoperatif vizüel aktivitede artış gözlenirken,6 gözde değişiklik saptanmadı. Arka kapsül opasifikasyonu en sık rastlanan postoperatif komplikasyon olup (18 gözde) diğerleri; fibrin reaksiyonu (8 göz), maküler ödem (12 göz), inflamasyon atağı (10 göz),postoperatif göz içi basınç artışı (6 göz), posterior sinesi (6 göz), anterior sinesi (4 göz), kapsül kontraksiyonu (2 göz), epiretinal membran (6 göz), iris capture (1 göz) gözlendi. 1 hastada endoftalmi görüldü. Sonuç: Katarakt cerrahisinin vizüel sonuçları üveitin etyolojisine ve arka segment tutulumunun varlığına göre değişir.Uygun hasta seçimi,uygun preoperatif hazırlık.dikkatli cerrahi ve postoperatif arka segmentin dikkatli ve sıkı takibi ile fakoemülsifikasyon ve intraokuler lens implantasyonu emniyetli ve etkili bir yöntemdir.
