Capsulorhexis in white cataracts using green-light endoillumination probe

Abstract

Purpose: To perform a safe and controlled capsulorhexis in white cataracts with no fundus reflex without using capsular dyes. Methods: After attaching a green filter into the endoillumination unit of the vitrectomy set and turning off the operating room and microscope lights, capsulorhexis was performed under the green light of the endoillumination probe, which was placed on the periphery of the cornea in a manner that it followed1 the edge of the capsulorhexis. 22 eyes of 20 patients (mean age: 53.7±13.2 years) with white cataracts operated between July 2003- May 2004 were included in the study. After the capsulorhexis, cataract surgery was continued in the usual fashion. Also, during the capsulorhexis and after the completion of the capsulorhexis, the capsulorhexis edge "Was photographed under the green light of the endoillumination probe. Results: A continuous curvilinear capsulorhexis was completed successfully in 20 eyes of the 22 cases (90.9%). In 2 cases with hypermature cataracts, capsulorhexis was completed, although with irregular borders, there were no radial tears. There were no postoperative complications in any oj the cases. Conclusion: With this new technique that we performed, a safe and controlled capsulorhexis in white cataracts can be done with no need for capsular dyes.

Amaç: Fundus reflesinin alınamadığı beyaz kataraktlarda kapsül boyaları kullanmadan güvenli ve kontrollü bir kapsüloreksis yapmak. Yöntem: Vitrektomi setinin endoillüminasyon ünitesine yeşil bir filtre yerleştirildikten ve mikroskop lambasıyla ameliyathane ışıkları söndürüldükten sonra, endoillüminasyon probu kapsüloreksis hattını takip edecek şekilde kornea periferine temas ettirilerek yeşil aydınlatma altında kapsüloreksis yapıldı. Temmuz 2003-Mayıs 2004 tarihleri arasında ameliyat edilen toplam 20 hastanın (yaş ortalaması; 53.7±13.2) 22 beyaz kataraktlı gözü çalışmaya dahil edildi. Kapsüloreksis sonrası katarakt cerrahisine fakoemülsifikasyon ile devam edildi. Ayrıca kapsüloreksis sırasında ve kapsüloreksis tamamlandıktan sonra, kapsüloreksis hattı, yeşil ışıklı endoillüminasyon probunun aydınlatması altında fotoğraflandı. Sonuçlar: Yirmiiki gözün 20'sinde (%90.9) düzgün kenarlı kapsüloreksis başarıyla tamamlandı. Hipermür kataraktı olan 2 gözde ise kapsüloreksis hattı düzensiz olmasına rağmen radyal yırtık gelişmeden kapsüloreksis tamamlandı. Olguların hiçbirinde ameliyat sonrası bir komplikasyona rastlanmadı. Yorum: Uyguladığımız bu yeni teknikle, beyaz kataraktlarda kapsüloreksis, kapsül boyalarına ihtiyaç duymadan güvenli ve kontrollü bir şekilde yapılabilir.
