Abstract

Purpose: Pseudocataract development with calcium and phosphate deposition in hydrophi­lic acrylic intraocular lens (IOL) as a result of unqualified production was noted in 5 cases from 2 ophthalmology clinics and the cases were presented herein to point out the subject. Methods: Intraocular lens exchange was performed in 4 of 5 pseudocataract cases with opacified IOLs who were referred to Uludağ University, Faculty of Medicine, Ophthalmology Department and Bahar Göz Merkezi with the complaints of decreased visual acuity following cataract operation at different clinics. Results: One of the three patients who was referred to Uludağ University, Faculty of Medi­cine, Ophthalmology Department with the complaint of decreased visual acuity was 57- year-old male who had insulin dependent diabetes for 20 years and had undergone cataract surdery one year before. The other patient was 53- year -old female with type 2 diabetes for 15 years and had glaucoma treated with timolol maleate. She was referred to the clinic with the complaint of decreased visual acuity following the cataract extraction 3 years before. The third case was a male patient who had undergone bilateral cataract surgery 5 years before and complained of pale vision in his right eye. One of the extracted IOL was Ophthalmed ACRL-C16 (SN: 00069-0405) which had been implanted in 2001. The brand of the other extracted IOL was unclear. One of the patients who was admitted to Bahar Göz Merkezi was 53- year- old female with type 2 diabetes. She had undergone cataract extraction and IOL (Aqua Sense) implantation 3 years before and her examination revealed visual acuity of 3 mfc. The other patient was a 80- year old male with a visual acuity of 2 mfc who had undergone cataract surgery for his right eye. Both cases under­went IOL exchange. All the extracted IOLs were acrylic. And the optics were particularly opaci­fied. The brands of only 2 IOLs were known. Conclusion: Three of the 5 pseudocataract cases were diabetic and 4 of them underwent IOL exchange. The relation of diabetes with psedocataract has been also implied by other aut­hors. One of the cases was followed up without IOL exchange as he had a relatively good visual acuity. The patients who undergo cataract extraction and hydrophilic acrylic IOL implantation, particularly the diabetics, should be followed up for at least 2 years after the surgery.

Özet

Amaç: Kaliteli üretim yapılmadığı takdirde, hidrofilik akrilik göziçi lenslerinde kalsiyum ve fosfat benzeri maddelerin birikmesi sonucu ortaya çıkan ve değişik iki klinikde rastlanan beş psödokatarakt olgusu bildirilerek konuya dikkat çekmek. Yöntem: Başka kliniklerde göziçi lens uygulaması geçiren ve görme azlığı yakınmasıyla 3'ü Uludağ Üni. Tıp Fak. Göz Hastalıkları AD ye ve 2'si Bahar Göz Merkezine başvuran 5 olgu­nun 5 gözünde göziçi lensinde beyazlaşma sonucu ortaya çıkan psödokatarakt nedeniyle 4'ünde GİL değişimi yapılmış ve sonuçlar değerlendirilmiştir. Bulgular: Değişik kliniklerde katarakt ameliyatı geçirmiş 5 olgudan görme azalması yakın­masıyla U.Ü Tıp Fak. Göz Hast. AD başvuran üç olgudan biri; 57 yaşında, kadın, 20 senelik dia-betli ve insülin tedavisi gören olgunun sağ gözünden bir sene önce, diğeri 53 yaşında kadın olgu ise 15 yıldır tip 2 diabetli ve glokomlu olup timolol maleat kullanmakta, 3 yıl önce geçirmiş ol­duğu katarakt ameliyatı sonrası görmesinin azalması nedeniyle GİL değişimi geçirmiştir. 3 üncü olgu ise sağlıklı erkek hasta olup 5 sene önce heriki gözünden katarakt ameliyatı geçirmiş ve sağ gözde soluk görme yakınması olan psödokataraktlı olgu idi. İlk olgudaki GİL Ophthalmed ACRL-C16 (SN: 00069-0405) marka olup 2001 yılında implante edilmiş, diğerininki ise bilin­miyordu. Bahar Göz Merkezine başvuran iki olgudan bir tanesi yine 3 yıl önce başka bir klinikte katarakt ameliyatı geçirmiş ve Aqua Sense GİL uygulaması yapılmış olan 53 yaşında kadın olgu tip 2 diabetli olup görmesi 3mps, diğeri ise 80 yaşında erkek olgu sağ gözde geçirmiş olduğu ka­tarakt ameliyatı sonrası görmesi 2 mps ye düşmüş bir olgu idi. Heriki olguda da GİL değişimi yapıldı. Tüm dört olguda çıkarılan GİL ler akrilik kökenli ve ikisinin üretici firması bilinen diğer ikisinin üreticisi meçhul olup hepsinde özellikle GİL gövdesi kesifleşmiş lenslerdi. Tartışma: Son iki sene içinde 2'si markası bilinen, 3'nünse saptanamamış GİL uygulaması sonucu ortaya çıkan ve birisinde de iyi görmesi nedeniyle izlenen 5 psödokatarakt olgusunun 3'ünde kaynaklarca da doğrulanmış, kontrol edilebilen diabetus mellitus mevcuttu. Bunlardan 4 olguya skleral fiksasyonlu GİL değişimi yapılmıştır. Bu nedenlere bağlı olarak, özellikle diabetli olanlarda hidrofilik akrilik GİL uygulanan olguların en az iki yıl izlenmesi gerektiği savunul­muştur.
