The results of primary posterior capsulorhexis and anterior vetrectomy combined with posterior optic capture on visual axis opacification in pediatric patients with cataract

Abstract

Purpose: To determine the effectiveness of primary posterior capsulorhexis (PCCC) combined with or without anterior vitrectomy and posterior optic capture (POC) on preventing se¬condary opacification of the visual axis and other complications requiring secondary interventions in pediatric cataract. Methods: In a prospective and randomized these study, PCCC combined with (Group II) or without anterior vitrectomy and POC (Group I) performed in pediatric cataract patients and results were compared to an age and gender matched pediatric cataract patients who did not have PCCC and anterior vitrectomy whose medical records were studied retrospectively (Group III). Results: In Group III, 58 eyes (92%) had a posterior capsular opacification followed by 14 eyes (48.3%) in Group I and 3 eyes (12.5%) in Group II. Thirty two eyes (50.8%) in Group III needed Nd-YAG laser posterior capsulotomy and 4 of them were further needed surgical capsu-lotomy-membranectomy because of visual axis opasification; 2 eyes required Nd-YAG laser posterior capsulotomy postoperatively in Group I and no eye in Group II needed a secondary procedure. Most of the Group III eyes developed significant complications (updrawn pupil, occ-lusio pupilla, peripheral anterior synechia, transient and permanent glaucoma and fibrinous uvei-tis). While no Group II eye developed a serious complication, some degree of fibrinous uveitis without a sequelae was noted in two eyes. Conclusion: PCCC combined with anterior vitrectomy and POC is the best method in dec¬reasing the frequency of visual axis opacification and providing a clear visual axis in pediatric cataract. Reopacification rate and occurrance of various complications after operations uncombi-ned with anterior vitrectomy and POC or without PCCC are higher.

Amaç: Pediatrik katarakt olgularında sekonder görme aksı opasifikasyonunun ve ikincil müdahale gerektiren diğer komplikasyonlarm önlenmesinde primer posterior kapsüloreksisin (PPK), tek başına veya anterior vitrektomi ve posterior optik capture (POC) ile kombine etkinliğini belirlemek. Yöntem: Prospektif ve randomize.bu çalışmada; pediatrik katarakt olgularında PPK, tek başına (Grup I) veya anterior vitrektomi ve posterior optik capture ile kombine olarak (Grup II) uygulandı. Sonuçlar PPK ve anterior vitrektomi yapılmamış, tıbbi kayıtları retrospektif olarak elde edilmiş, yaş ve cinsiyetleri benzer pediatrik katarakt olguları (Grup III) ile karşılaştırıldı. Sonuçlar: Grup IU'de 58 (%92) gözde, Grup I'de 14 (%48.3) gözde, Grup H'de 3 (%12.5) gözde posterior kapsül opasifikasyonu izlendi. Grup IU'de 32 (%50.8) gözde Nd-YAG laser posterior kapsülotomi gereksinimi oldu ve daha sonra görme aksı opasifikasyonu nedeniyle bunlardan 4'üne cerrahi kapsülotomi-membranektomi uygulandı; Grup I'de 2 göze postoperatif Nd-YAG laser posterior kapsülotomi gereksinimi oldu; Grup H'de ise hiçbir göze ikincil girişim yapılmadı. Grup IU'de gözlerin çoğunluğunda pupil ektopisi, oklüzyo pupilla, periferik anterior sinesi, geçici veya kalıcı glokom ve fibrinöz üveit gibi önemli komplikasyonlar gelişti. Grup II'de önemli bir komplikasyon görülmemekle birlikte 2 gözde çeşitli derecelerde sekelsiz fibrinöz üveit saptandı. Tartışma: Pediatrik katarakt olgularında, açık bir görme aksı sağlamak ve görme aksı opasifikasyonu sıklığını azaltmak için anterior vitrektomi ve POC ile kombine PPK en iyi yöntemdir. PPK uygulanmayanlarda veya anterior vitrektomi ve POC ile kombine edilmemiş PPK lı olgularda reopasifikasyon ve postoperatif çeşitli komplikasyonlarm oluşması daha sıktır.
