Amniotic membrane transplantation in nontraumatic corneal perforations

Abstract
Amniotic Membrane Transplantation in Nontraumatic Corneal Perforations Purpose: To evaluate the efficacy of amniotic membrane transplantation in supporting the glob entegrity in perforations associated with corneal diaseases. Method: Twenty-eight patients treated with amniotic membrane transplantation because of nontraumatic corneal perforations were assessed prospectively from 2002 to 2004. Eighteen male (64.3%) and 10 female (35.7%) patients with a mean age of 40.5 ± 1.2 years (range 8 months - 80 years) were included in the study. They were followed up for an average of the 10,5 months (range 3-18 months). Single or multilayered amniotic membrane was placed on the corneal surface with epithelial side up, exceeding 2 to 3 mm from the limbus and secured with 10-0 nylon sutures to the perilimbal episclera. Result: The patients were grouped according to the underlying causes for perforation: 10 patients (35.7%) with sterile corneal ulcer, 5 patients (17.8%) with bacterial corneal ulcer, 3 patients (10.7%) with alkaline burns, 2 patients (7.2%) with herpetic keratitis and 2 patients (7.2%) with ocular surface disorder associated with rheumatoid arthtitis. Amniotic membrane transplantation was applied twice in 4 patients, three times in 2 patients and once in the other 22 patients. Penetrating keratoplasty was performed in three patients two months after amniotic membrane transplantation and in one patient one month after amniotic membrane transplantation. Graft abscess was developed in one patient leading to corneal perforation. We applied amniotic membrane transplantation in this patient for the secondgime. Conclusion: Amniotic membrane transplantation is an effective method for the treatment of nontraumatic corneal perforations. It can be used as either a permanent therapy or as a temporary measure until penetrating keratoplasty is performed.

Amaç: Kornea hastalıklarına bağlı perforasyonlarda, glob bütünlüğünün korunmasında amniyon zarı transplantasyonun etkilerini araştırmak. Gereç ve yöntem: Ankara Eğitim ve Araştırma Hastanesi Göz Kliniği Kornea bölümünde 2002-2004 tarihleri arasında, travmatik olmayan kornea hastalıklarına bağlı perforasyon nedeniyle amniyon zarı transplantasyonu yapılan 18'i (%64,3) erkek, 10'u (%35,7) kadın 28 hasta prospektif olarak değerlendirildi. Hastaların yaş ortalaması 40,5 ±1.2 yaş ( 8 ay-80 yaş) ve ortalama takip süreleri 10,5 ay (3-18 ay) idi. Tek kat veya çok katlı amniyon zarı kornea yüzeyine limbus sınırlarını 2-3 mm. geçecek ve epitel yüzü yukarı gelecek şekilde yerleştirildi ve 10-0 nylon ile perilimbal episkleraya sütüre edildi. Bulgular: Perforasyon nedenleri; 10 hastada (%35,7) steril kornea ülseri, 5 hastada (%17,8) bakteriyel kornea ülseri, 3 hastada (%10,7) alkali yanık, 2 hastada (%7,2) herpetik ke-ratit, 2 hastada (%7,2) romatoid artrite bağlı oküler yüzey bozukluğu, 6 hastada (%21,4) ise desmatosel idi. Hastaların 4'üne 2 kez, 2'sine 3 kez, diğer 22'sine bir kez amniyon zarı transplantasyonu yapıldı. Üç hastada amniyon zarı transplantasyonundan 2 ay sonra, 1 hastada amni-yotik membran transplantasyonundan 1 ay sonra penetran keratoplasti uygulandı. Bu hastalardan birinde yapılan penetran keratoplasti sonrası gelişen greft absesine ikincil perforasyon nedeniyle 2. kez amniyon zarı transplantasyonu yapıldı. Sonuç: Amniyon zarı transplantasyonu; travmatik olmayan kornea perforasyonlarının tedavisinde, kalıcı veya terapötik keratoplasti öncesi geçici olarak uygulanabilecek etkili bir yöntemdir.
