Abstract

Object: Investigation of techniques in cataract surgery which were described in a book about surgery written in Turkish in XVth century. Anatolia. Material And Method: The 13*, 15th, 16th, 18th, 22nd and 23rd chapters of the book "Cer-rahiyat-ül Haniyye", which was written by a physician named Şerefeddin Sabuncuoğlu, is concerned with ocular diseases. In the 23rd chapter, surgical treatment of the humor which has flowed down into the eye is described. This chapter has been reviewed in detail and the relationship between the method which was used in the past and the stages of transition to current practice has been reviewed. Results: Scleral limbus is entered from behind by means of "mikdah", the lens with cataract is removed and the humour which has flowed down into the eye (cataract) is eliminated by the help of "mil" (probe). Following this stage, the eye is washed whit salt water. After being dressed, the eye covered and the patient is made to lie in a quiet room without moving. Discussion: In both past and current practices, knowledge and skill plays an important part and it seems that some modern practices may have been inspired by ancient manoeuvres.

Özet

Amaç: XV. Yüzyılda Anadolu'da Türkçe yazılmış bir cerrahi kitabında geçen göz bahislerinden katarakt cerrahisine ait tekniklerin incelenmesi. Gereç ve Yöntem: Şerefeddin Sabuncuoğlu adlı hekim tarafından yazılmış Cerrahiyetül Haniyye kitabının 13, 15,16, 18, 22 ve 23. cü bahisleri göz hastalıkları ile ilgilidir. 23.cü fasılda göze inen suyun (katarakt) cerrahi tedavisi anlatılmaktadır. Bu fasıl detayıyla incelenmiş, geçmiş dönemlerdeki yöntemle bugünkü uygulamalara geçiş aşamaları arasındaki ilintiler gözden geçirilmiştir. Bulgular: Skleral limbus gerisinden "mikdah" ile girerek kataraktlı lens lükse edilmesinin yanı sıra ayrıca göze inen su (katarakt) "mil" yardımı ile temizlenir. Bu aşamadan sonra göz tuzlu su ile yıkanıp temizlenir. Pansuman yapılarak kapatılır ve hasta sessiz ve karanlık bir odada uzun süre hareketsiz ve sakin bir şekilde yatırılır. Yorum: Gerek geçmiş dönemlerdeki uygulamalar gerek günümüz uygulamalarında, bilgi* ve becerinin ön plana çıktığı sonucunu ortaya koymasının yanısıra, bazı modern uygulamaların çok eski dönemlerdeki manevralardan ilham almış olabileceği izlenimi edinilmiştir.
