Surgical approach for the orbital implant exposure

Abstract

In this paper, treatment modalities for cases presenting with orbital implant exposure and their results are presented. The study included 5 patients that applied to our oculoplastic service with an orbital implant exposure between the date of 2003-2005. Three cases had underwent evisceration surgery and 2 cases had underwent enucleation surgery. There were 4 males and 1 female and the mean age was 47 years (range, 28-71 years). In 4 cases there were porous implants, in 1 case there was an acrylic implant. In 3 of the cases 2 months after surgery, in 1 case 4 years after surgery, in 1 case 8 years after surgery an orbital implant exposure was detected. The mean interval between the exposure and the revision was 10 months (range, 1-24 months).In I case the implant was extracted and due to patient's will, without another implant insertion a revision by tenon capsule and conjunctiva was performed. la-case which an acrylic sphere exposure occurred, a barrier was made by suturing the scleral flebs in front of the implant. In one case which an enucleation and implantation of dexon covered alumina implant had been performed, the same implant was inserted backward as possible as and closed by tenon capsule and conjunctiva. In two cases which had underwent evisceration and a porous implant had been inserted were performed posterior sclerotomy and secondary orbital implantation. During the follow- up of all cases, a new exposure were not observed.

Özet

Bu çalışmanın amacı orbita implantı açılımı ile başvuran olgularda cerrahi yaklaşımlarımızı takdim etmek ve sonuçlarımızı tartışmaktır. 2003 -2005 yılları arasında implant açılımı ile oküloplasti polikliniğimize başvuran 5 hasta çalışma kapsamına alındı. Üç olgu eviserasyon, 2 olgu enükleasyon cerrahisi geçirmişti. Hastaların 4'ü erkek,l'i kadın olup yaş ortalamaları 47 (28 -71 yıl) idi. Dört olguda poröz, 1 olguda akrilik implant mevcuttu. Olgulardan üçünde ameliyat sonrası ilk 2 ayda, birinde 4., diğerinde ise 8. yılda açılım tesbit edilmişti. Açılma ve revizyon arasındaki süre ortalama 10 ay (1 -24 ay) idi. Bir olguda implant çıkarıldı ve hastanın isteği üzerine başka implant takılmadan tenon ve konjonktiva kapatıldı. Akrilik sfer açılması olan diğer bir olguda, hazırlanan sklera flepleri imp lant önünde sütüre edilerek bariyer oluşturuldu. Enükleasyon yapılmış ve dexon kaplı alumina implant yerleştirilmiş bir olguda ise aynı implant mümkün olduğunca geriye doğru yerleştirile rek tenon ve konjonktiva kapatıldı. Eviserasyon yapılıp, poröz implant yerleştirilmiş iki olguda ise posterior sklerotomi ve sekonder sfer implantasyönu yapıldı. Olguların tümünde izlemler boyunca yeniden açılma gözlenmedi.
