Efficacy and side effects of the fixed combination of latanoprost and timolol (xalacom) in glaucoma treatment (preliminary results)

Abstract
Purpose: Investigating the efficacy and safety of the fixed combination of Latanoprost 0.005% and Timolol 0.5% (Xalacom) in lowering intraocular pressure (IOP) in glaucoma patients that are not treated previously or having IOP not adequately controlled with antiglaucomatous monotherapy. Materials and Methods: 49 eyes of 27 patients that are not treated previously or having IOP not adequately lowered with monotherapy were included in4he study.Study patients consisted of 15 patients with primary open angle glaucoma (POAG), 6 patients with pseudoexfolia-tive glaucoma (PEXG), 5 patients with ocular hypertension (OHT) and 1 patient with pigmentary glucoma. The mean follow- up duration of the cases was 2 months. Patients were administered the fixed combination of Latanoprost and Timolol (Xalacom) once a day (at 08.00 pm). Applanation tonometer was used for IOP measurements. For IOP measurements, the values before treatment and at 1st and 2nd month after treatment were determined. Ocular and systemic side effects were investigated. The results were compared statistically. Results: The mean IOP value was 21.02 ± 5.3 mmHg at baseline, 13.3 ± 3.1 mmHg at 1st month and 12.9 ± 2.7 mmHg at 2nd month after treatment. The rate of decline in baseline IOP value with the combination of Latanoprost and Timolol was 36.7% at 1st month and 38.6% at 2 nd month. With these results the rate of decline in IOP was statistically significant when the baseline and following IOP values were compared. (Paired t test, p < 0.01). The most common ocular side effect was irritation with the rate of 7.4%. Conclusion: Adequate IOP decline was achieved in glaucoma treatment by using the fixed combination of Latanoprost and Timolol (Xalacom) alone. No serious treatment- related side effect was observed with combination during the follow-up period and it was well tolerated.

Amaç: Daha önce ilaç kullanmamış veya antiglokomatöz mono terapi ile göziçi basıncı (GİB) yeterli kontrol edilemeyen glokom hastalarında, Latanoprost % 0,005 ve Timolol %0,5 sabit kombinasyonunun (Xalacom) GİB düşürmedeki etkinliğinin ve güvenilirliğinin araştırılması. Gereç ve Yöntem: Önceden ilaç kullanmayan veya mono terapi ile yeterli GİB düşüşü sağlanamamış 27 hastanın 49 gözü çalışma kapsamına alındı.Hastaların 15'inde Primer açık açılı glokom (PAAG), 6'sında Psödoeksfoliyasyon glokomu ( PEXG), 5'inde Oküler hipertansiyon (OHT), l'inde Pigmenter glokom mevcuttu. Olgular ortalama 2 ay takip edildiler. Hastalara günde 1 kez (saat 20.00 de) olmak üzere Latanoprost + Timolol sabit kombinasyonu (Xalacom) uygulandı. GİB ölçümleri applanasyon tonometresi ile yapıldı. GİB ölçümünde tedavi öncesi, tedavi sonrası 1. ay ve 2. aydaki değerler saptandı. Oküler ve sistemik yan etkiler araştırıldı. Sonuçlar istatistiksel olarak karşılaştırıldı. Bulgular: Başlangıç GİB değeri ort. 21.02 ± 5.3 mmHg, 1. ay GİB değeri ort. 13.3 ± 3.1 mmHg, 2.ay GİB değeri ort. 12.9 ± 2.7 mmHg olarak saptandı. Latanoprost + Timolol kombinasyonu tedavisi ile başlangıç GİB değerine göre 1. ayda %36.7 oranında, 2.ayda ise %38.6 oranında GİB düşüşü sağlandı. Bu sonuçlarla başlangıç GİB ve sonraki GİB değerleri karşılaştırıldığında, GİB'da saptanan düşüş miktarı istatistiksel olarak anlamlıydı (Eşleştirilmiş t testi, p < 0.01). En fazla rastlanan oküler yan etki %7.4 ile irritasyon oldu. Yorum: Tek başına Latanoprost + Timolol sabit kombinasyonu (Xalacom) kullanımı ile glokom tedavisinde, yeterli oranda GİB düşüşü elde edildi. İzleme süresi boyunca kombinasyona ait ciddi bir yan etkiye rastlanmadı ve iyi tolere edildiği görüldü.
